Chlamydia trachomatis infection of the neovagina in transgender women

Abstract: We report two cases of neovaginal Chlamydia trachomatis infection in transgender women who underwent penile-inversion vaginoplasty procedures with integrated peritoneum and urethral grafts. These tissue types may have facilitated C. trachomatis infection. Medical providers should implement neovaginal screening for bacterial STIs in transgender patients at risk for infection.

“…Neovaginal STIs have infrequently been reported in the literature and include HSV and HPV/genital warts in penile-inversion vaginoplasty, C. trachomatis in procedures that involved penile skin and grafts with urethra mucosa or abdominal peritoneal lining (353), and N. gonorrhoeae in both penile-inversion and colovaginoplasty (354)(355)(356)(357)(358)(359). If the vaginoplasty used an intestinal graft, a risk also exists for bowel-related disease (e.g., adenocarcinoma, inflammatory bowel disease, diversion colitis, and polyps) (360)(361)(362).…”

“…Whereas HPV has been associated with penile and scrotal skin, other STIs such as chlamydia or gonorrhoea have a predilection for columnar and transitional epithelium and have been identified in transgender women following GAS with colonic mucosa and integrated urethral or peritoneal grafts. 9,10 Our case report focuses on a transgender woman who underwent vaginoplasty with the penile skin inversion technique and was subsequently diagnosed with neovaginal warts. This case provides evidence for the clinical utility of TCA in a transgender woman with neovaginal warts, and particularly speaks to its functionality when patient-applied treatments are not feasible, or when an operative procedure is not desired or is contraindicated.…”
