Chronic Lateral Ankle Instability: Topical Review

Abstract: Chronic ankle instability can result from untreated or badly managed acute lateral ankle ligament injuries. Conservative management is the modality of choice for acute lateral ankle ligament injuries, and operative treatment is reserved for special cases. Failure after strict rehabilitation may be an indication for surgery. Several operative options are available, including anatomic repair, anatomic reconstruction, and tenodesis procedures. Anatomic repair can be performed when the quality of the damaged ligam… Show more

“…Nowadays, conservative rehabilitation and surgery after ineffectual conservative management are the primary treatment for CAI (Yeo et al, 2016;Kim et al, 2019). However, some patients fail to recover or return to sports as usual (Doherty et al, 2017;Aicale and Maffulli, 2020). Hence, it will be beneficial to explore the pathophysiological alterations underlying CAI to improve its interventions.…”

“…The modified Broström procedure (repair of the anterior talofibular ligament and augmentation with the inferior extensor retinaculum, Fig. 1 ) is a standard procedure for CLAI patients who do not respond to three to six months of conservative management [ 4 6 ]. Patients with OLTs less than 15 mm in diameter are usually treated with intra-articular debridement and microfracture [ 1 , 7 ].…”

“…Approximately 40% of patients with ankle sprains experience repeated sprains, “giving way,” and other symptoms associated with chronic ankle instability (CAI) [ 2 ]. A well-structured functional and prophylactic rehabilitation program should be provided to those with CAI, and operative repair used only if rehabilitation has failed [ 3 ]. Females have been found to be more likely to report sprains and to suffer CAI than males [ 4 , 5 ].…”
