Chronic neuropathic pain after traumatic peripheral nerve injuries in the upper extremity: prevalence, demographic and surgical determinants, impact on health and on pain medication

Abstract: Background and aimsAside from the long term side effects of a nerve injury in the upper extremity with devastating consequences there is often the problem of chronic neuropathic pain. The studies concerning the prevalence of persistent pain of neuropathic origin after peripheral nerve injuries are sparse. The prevalence and risk factors associated with chronic neuropathic pain after nerve injuries in the upper extremity were assessed.MethodsA standardized data collection template was employed prospectively and… Show more

“…In some cases, axonotmesis lesions also benefit from wrapping with nerve conduits to accelerate nerve regeneration [7]. However, the prognosis of surgical nerve repair to solve PNI remains suboptimal, regardless of the site of injury or the chosen therapeutic technique [11].…”

“…Major nerve injury, younger age, and less time from surgery were predictors for pain after traumatic nerve injuries in the upper extremities. 26 Unfortunately, earlier studies could not detect a specific pattern using quantitative sensory testing (QST) or structural parameters using skin biopsy that would separate the neuropathy patients into painful and nonpainful individuals. 18,21,23,25,38 Impaired conditioned pain modulation (CPM) is an indication of altered descending inhibitory modulation of pain 4,39 and offers the opportunity to test whether patients with neuropathic pain have signs of increased pain sensitivity or abnormal CPM Sponsorships or competing interests that may be relevant to content are disclosed at the end of this article.…”

“…Peripheral nerve injuries (PNIs) result in partial or complete disruption of normal nerve physiology. Therefore, PNI represents a health problem causing a socioeconomic burden and a poor quality of life for patients [ 1 ]. Then, new treatments for PNI aim to diminish iatrogenic effects and promote axonal regeneration and functional recovery as wholly and promptly as possible.…”
