Chylothorax Treated Via Thoracic Duct Ligation and Omentalization

Abstract: Chylothorax is an uncommon, potentially life-threatening disease of dogs and cats. Medical records of 12 animals (five dogs and seven cats) undergoing surgical management of chylothorax from 2001 to 2005 were reviewed. All animals received thoracic duct ligation and thoracic omentalization. In some cases, a combination of subtotal pericardectomy and/or pleural stripping was also employed. All animals survived surgery, and none was lost to follow-up. Median survival time for cats was 209 days (range 2 to 1328 d… Show more

“…The omentum has a number of unique physiological properties including promoting angiogenesis, enhancement of inflammatory functions, macrophage margination, immune function and fibroplasia that have been exploited in small animal surgery (Hosgood 1990, White 2000, Hoelzler and others 2001, Johnson and Mann 2006, McAlinden and others 2009, Stewart and Paggett 2010, DaSilva and others 2011, Bussadori and others 2011, Franklin and others 2011). In particular, its ability to contain, resolve and wall off sites of abscessation has proved useful in the management of abscesses in the mediastinum (Franklin and others 2011), pancreas (Johnson and Mann 2006) and prostate gland (White 2000).…”

“…Postoperative pleural infection associated with early mortality in animals managed surgically for chylothorax has also been reported in the veterinary literature in at least one study. Stewart and Padgett () reported death in one of five dogs treated surgically for chylothorax, and postmortem examination confirmed acute fibrinopurulent pleuritis. It is interesting to note this study also used a combined median sternotomy and midline coeliotomy to approach the thorax and allow thoracic omentalization.…”

“…[39][40][41][42][43] Most recommendations for treatment include a combination of the following potential surgical options: thoracic duct ligation, pericardectomy, omentalization of the thoracic cavity, and cisterna chyli ablation. Arrhythmias that require treatment are occasionally encountered.…”
