The treatment of vulvovaginal candidiasis (VVC) due to Candida glabrata is challenging, with limited therapeutic options. Unexplained disappointing clinical efficacy has been reported with systemic and topical azole antifungal agents in spite of in vitro susceptibility. Given that the vaginal pH of patients with VVC is unchanged at 4 to 4.5, we studied the effect of pH on the in vitro activity of 11 antifungal agents against 40 C. glabrata isolates and compared activity against 15 fluconazole-sensitive and 10 reduced-fluconazole-susceptibility C. albicans strains. In vitro susceptibility to flucytosine, fluconazole, voriconazole, posaconazole, itraconazole, ketoconazole, clotrimazole, miconazole, ciclopirox olamine, amphotericin B, and caspofungin was determined using the CLSI method for yeast susceptibility testing. Test media were buffered to pHs of 7, 6, 5, and 4. Under conditions of reduced pH, C. glabrata isolates remained susceptible to caspofungin and flucytosine; however, there was a dramatic increase in the MIC 90 for amphotericin B and every azole drug tested. Although susceptible to other azole drugs tested at pH 7, C. albicans strains with reduced fluconazole susceptibility also demonstrated reduced susceptibility to amphotericin B and all azoles at pH 4. In contrast, fluconazole-sensitive C. albicans isolates remained susceptible at low pH to azoles, in keeping with clinical observations. In selecting agents for treatment of recurrent C. glabrata vaginitis, clinicians should recognize the limitations of in vitro susceptibility testing utilizing pH 7.0.

STRESZCZENIEGrzybice są schorzeniami bardzo często spotykanymi w codziennej praktyce dermatologicznej, a znajomość licznych preparatów przeciwgrzybiczych, jakimi dysponuje współczesna medycyna, jest przydatna w każdej niemal specjalności lekarskiej. Rozpoczęcie leczenia zakażeń grzybiczych powinno być poprzedzone prawidłowo postawioną diagnozą opartą na wywiadzie, dokładnym badaniu klinicznym i diagnostycznych badaniach mikologicznych. Ustalenie właściwego rozpoznania grzybicy jest więc możliwe wtedy, gdy będzie ono potwierdzone prawidłową diagnostyką laboratoryjną przeprowadzoną w pracowni mikologicznej, gdyż głównym kryterium upoważniającym do rozpoznania grzybicy i rozpoczęcia terapii jest wyizolowanie i identyfikacja grzyba od pacjenta, który ma być leczony. W prezentowanym przeglą-dzie dotyczącym leczenia najczęściej spotykanych w praktyce dermatologicznej zakażeń grzybiczych zaproponowano leki przeciwgrzybicze najbardziej znane i nadal w naszym kraju wykorzystywane, których przydatność w zastosowaniu klinicznym potwierdziły wieloletnie obserwacje. Mamy nadzieję, że przedstawione propozycje leczenia najczęściej spotykanych powierzchownych zakażeń grzybiczych będą przydatne w codziennej praktyce lekarskiej. ABSTRACTFungal infections are very commonly occurring diseases in everyday dermatological practice, and knowledge of the wide spectrum of antifungal preparations available nowadays is helpful in almost all medical disciplines. The treatment of fungal infections should be preceded with a proper diagnosis based on the patient's history, precise clinical examination and diagnostic mycological procedures. A correct diagnosis of fungal infection is possible by its confirmation with proper investigations carried out in a mycological laboratory. The main criteria of diagnosis of fungal infection justifying its therapy is isolation and identification of the fungal pathogen from the patient. In the review of the treatment of the fungal infections most commonly occurring in dermatological practice, the best known antifungals currently used in our country, whose usefulness in clinical practice has been confirmed

Statistical figures represent that fungal infections are the major cause of thousands of deaths each year. Despite the availability of treatment options invasive and systemic fungal infections have alarmingly higher mortality rates. This current review tabulates such fungal organisms that are pathogenic and fatal to human. This article provides a compilation of all Mycoses in human and also reports about the risk group of each fungus based on which, the level of risk caused by fungi can be determined. Infections caused by Dimorphic fungal pathogens (Blastomyces, Coccidioides, Marneffei) and Dematiaceous hyphomycetes (Cladophialophora, Rhinocladiella) belong to major fatal causing risk group 3 resulting in higher death rates of individuals. It also reports about various types of possible classification of mycoses i.e. based on reaction of human to fungi, host susceptibility, pathogen type, body location and risk group. The article highlights the estimated incidence of fungal diseases due to these pathogens, but these statistical figures are usually underestimated so there is a great need for advancement in understanding of fungus, their resistance along with immunity reports. In order to meet all these disease combating requirements, many high quality research initiatives with proper sources of funding and encouragement have to be provided. Article also reviews the major antifungal drugs available along with their brief pharmacokinetic profile. It also briefs some take home remedies in order to prevent and treat major fungal infection along with the alternative and novel modelling techniques developed recently for drug designing to target various development stages of fungal organism.
