Cidofovir Gel as Treatment of Follicular Spicules in Multiple Myeloma

Abstract: Follicular spicules in MM are probably not caused by the trichodysplasia spinulosa-associated virus. Merkel cell polyomavirus could contribute to the origin of this dermatosis.

“…In contrast to the low copy numbers of MCPyV detected by van Boheemen et al 4 in their FSMM case, TSPyV is found at an average of about 10 6 copies/cell in cases of TS 3 . Moreover, while ultrastructural analyses in cases of FSMM have consistently failed to identify viral particles in affected tissue, the overwhelming majority of cases of TS in which electron microscopy has been attempted do show evidence of viral particles 2 .…”

“…Van Boheemen et al 4 also argue that the responsiveness of the eruption to cidofovir gel supports a viral role for FSMM. However, the concurrent treatment of the patient’s multiple myeloma with systemic agents mitigates any conclusions regarding cidofovir’s direct effects.…”

“…Dear Editor , A possible contributory role of Merkel cell polyomavirus (MCPyV) in the formation of facial follicular spicules in multiple myeloma is debated . Here we report a case of chronic, widespread follicular papules with spicules and scalp hair loss in an immunocompromised host following double lung transplantation.…”

“…Histopathologically, specimens demonstrate abnormal maturation of hair follicles with inner root sheath differentiation containing excessive amounts of trichohyalin and intraepithelial viral inclusions. The differential diagnosis includes follicular spicules in multiple myeloma, which it has been suggested has a viral etiology that is not due to a polyomavirus .…”
