Cigarette Craving, Smoking Withdrawal, and Clonidine

Abstract: Clonidine, an alpha-2-adrenergic agonist, significantly reduces opiate withdrawal. Fifteen heavy smokers abstained from cigarettes on three separate occasions and received instead clonidine, placebo, or the benzodiazepine alprazolam. Clonidine and alprazolam diminished withdrawal symptoms. The two drugs suppressed anxiety, tension, irritability, and restlessness equally but clonidine had a greater effect than alprazolam on cigarette craving. These observations suggest that noradrenergic activity is a common fe… Show more

“…An increase in brain noradrenergic activation is common in many withdrawal syndromes [205] and anxiety states. Clonidine has successfully been used to control symptoms in a number of withdrawal states such as opioid [206], benzodiazepine [207], alcohol [208] and tobacco [209][210][211]. Clonidine also controls manic symptoms [212] and has been used in hyperactive children [213].…”

“…On the other hand, the combined sedative, anxiolytic and sympatholytic properties of clonidine have been used successfully in the therapy of various neuropsychiatric disorders associated with increased sympathetic neuronal activity, such as opiate withdrawal (Gold et al, 1978), cigarette craving and withdrawal symptoms in smokers (Glassman et al, 1984), and generalized and panic anxiety (Redmond, 1982). Also in the case of medetomidine, at least in normal subjects without pathological anxiety, the sedative and hypotensive drug actions seem to manifest in the same dose range, with a single 25 ,ug intravenous dose causing statistically significant sedation and reduction of BP.…”

“…People have complained of intense desires or cravings when they abstain from smoking. Several investigations have implicated craving as a factor in relapse, withdrawal, or stress in smokers (Glassman, Jackson, Walsh, Roose, & Rosenfeld, 1984;Shiffman & Jarvik, 1976). Utilizing aversion therapy for craving of cigarettes, alcohol, or other substances may provide a component that has been missing from previous interventions.…”
