Cigarette Smoking and the Risk of Supraventricular and Ventricular Tachyarrhythmias in High‐Risk Cardiac Patients with Implantable Cardioverter Defibrillators

Abstract: In patients with ischemic left ventricular dysfunction, continued cigarette smoking is associated with a significant increase in the risk of life-threatening ventricular tachyarrhythmias and inappropriate ICD shocks induced by rapid supraventricular arrhythmias. Our findings stress the importance of complete smoking cessation in this high-risk population.

“…Further, nicotine has been shown to stimulate the sympathetic nerves and markedly elevate serum catecholamine concentration, which is potentially arrhythmogenic (18). Smoking is indeed a significant risk factor for appropriate ICD shocks among patients with heart failure with an ICD (19)(20)(21). However, it is not certain whether smoking by itself is a risk for idiopathic Vf.…”

“…[10][11][12][13][14] Cigarette consumption is directly related to increased rates of sudden cardiac death (SCD), mainly through its direct toxic effects. Recently, the impact of the smoking status on SCD was studied in patients with established coronary artery 15 In that study, current cigarette smoking was a powerful independent predictor of the SCD risk and those that quit smoking experienced a significant reduction in the SCD risk.…”

“…Thus, more delicate procedures during AF ablation may be needed in patients that smoke and have large substrate changes. Second, continuing smoking has been proven to be an important risk factor for increasing SCD 15,16 and tachyarrhythmias in patients with implantable cardiovector defibrillators. Therefore, cessation of smoking should be encouraged in patient with AF, no matter whether they receive an invasive procedure to treat their AF or not.…”

“…Age younger than 70 years, nonischemic cardiomyopathy, non-use of statins, smoking, and interim appropriate ICD shocks were reported to be independent predictors of inappropriate ICD shocks. 16,24,29 The MADIT II trial data analysis as well as other recent studies shows a significant 3-fold increase in the risk of inappropriate ICD shocks among current smokers. The overall risk of inappropriate ICD therapy was significantly increased among current smokers (20%) compared to past smokers (14%) and patients who never smoked (11%).…”

“…1,24 In some cases the treatment for ventricular arrhythmias can precipitate AF and initiate an inappropriate ICD discharge. An inappropriate therapy itself causing VT can lead to an appropriate ICD discharge as well.…”
