A 34-year-old female presented with unilateral, localized, linear, and brown lesion over the left upper eyelid causing cosmetic blemish. On clinical examination, upturning of the eyelash under the involved skin was seen and diagnosed as a case of cilium incarnatum externum. A simple excision followed by electrolysis for this unusual abnormality provides satisfactory results for both patient and the treating physician. Only 9 cases have been reported till date in world literature.
