Circadian Rhythm of Salivary Cortisol, 17α-Hydroxyprogesterone, and Progesterone in Healthy Children

Abstract: Very few reference intervals for salivary steroids in children have been established to date (1 ). Even the manufacturers of salivary steroid assays do not provide sufficient reference data for their products. This lack of information is surprising because the measurement of salivary steroids has been accepted as being noninvasive and stress-free (2, 3 ). In particular, psychiatric and neuroendocrinologic experiments are frequently designed with saliva as the medium of choice for steroid analysis (4 -6 ).A lar… Show more

“…Given the short half-life of cortisol in saliva, concentrations of salivary cortisol reveal only momentary stress. This combined with the highly variable and reactive nature of the HPA axis, dependent on both psychological and physical factors, [16][17][18] makes it difficult to investigate a stable trait in the cortisol response. 19 It has recently become feasible to measure cortisol levels in hair over time.…”

“…Notably, there is some evidence that gender-specificity of adrenocortical activity evolves during pubertal development and is absent in young children (Kiess et al, 1995;Groschl et al, 2003;Netherton et al, 2004;Tegethoff et al, 2011b). In line with this, we did not reveal gender differences in the CAR of infants.…”

“…This phenomenon has been described as 'fetal programming' (Phillips and Jones, 2006), a specification of the more general concept of 'programming', which describes the process by which a stimulus or insult at a critical stage of development has lasting or lifelong significance (Lucas, 1991). The well-documented circadian rhythm of cortisol emerges within the first months postpartum (Price et al, 1983;Santiago et al, 1996;Antonini et al, 2000;Groschl et al, 2003).…”

“…The wide ranges in the results of salivary cortisol determinations reflect the high inter-individual variability, as reported. 9 Salivary cortisol determinations could be a tool for the diagnosis of adrenal insufficiency in these infants, although further studies are necessary to confirm these findings. There is a rate of normal NBIs whose serum and salivary cortisol levels are low, which renders the diagnosis of adrenal insufficiency difficult if only this measurement is taken into account.…”
