Circadian Variability of Fibrinolytic Markers and Endothelial Function in Patients with Obstructive Sleep Apnea

Abstract: The presence of obstructive sleep apnea adversely affects circadian fibrinolytic balance with higher mean plasminogen activator inhibitor-1 activity and antigen, and significantly lower mean tissue-type plasminogen activator activity compared with controls. This perturbation may be an important mechanism for increased cardiovascular events in patients with obstructive sleep apnea. Intermittent hypoxia and changes in circadian clock gene activity in obstructive sleep apnea may be responsible for these findings … Show more

“…However, the concomitant elevation of PAI-1 and the interaction between plasminogen activators and PAI-1 leads to the formation of an enzyme-inhibitor complex, which inhibits plasminogen activation activity. 11 Many authors suggest a protective role of TGF-␤ on the cardiovascular system because low levels of this molecule are related to coronary artery disease with a poor prognosis. 22,29,30 TGF-␤ has been shown to induce PAI-1 synthesis in several cell systems.…”

“…9,10 In recent years, coagulation in OSAS has received some attention, and different studies have focused on the fibrinolytic activity and the role of the plasminogen system in subjects with sleep apnea. [11][12][13][14][15] There are 2 forms of plasminogen activators: tissue-type (tPA) and urokinase-type (uPA). Both are serine proteases that convert plasminogen to the active enzyme plasmin.…”

“…First, disturbed sleep may result in more nocturnal light exposure, which can adjust the circadian system . Second, OSA can augment the transcription of circadian proteins . Third, there is some evidence in humans and other animals to suggest that hypoxia impacts both the SCN and melatonin secretion .…”

“…(41) Second, OSA can augment the transcription of circadian proteins. (42) Third, there is some evidence in humans and other animals to suggest that hypoxia impacts both the SCN and melatonin secretion. (43) OSA has not been definitively shown to cause circadian disruption, but it has the potential to cause internal desynchrony and disruption of the day/night rhythm of BTMs, which could impact bone health.…”
