Circumferential Pelvic Antishock Sheeting: A Temporary Resuscitation Aid

Abstract: Acute traumatic pelvic ring instability causes severe pain and associated hemorrhage. Circumferential pelvic sheeting provides patient comfort and noninvasive, rapid, and temporary pelvic ring stability. A bed sheet is readily available, inexpensive, easily applied around the pelvis, and disposable.

“…Staff working in trusts that did not have these available indicated they used bed sheets secured with cable ties, tape or artery clips in order to provide pelvic compression and stability. Whereas this technique has been well described [13], it does not allow the applied reduction force to be controlled to a predetermined level [14] and is less effective in reducing symphyseal diastasis in open-book injuries when compared with pelvic binders [15]. Also, a narrow sheet may increase the risk of developing pressure sores when compared with a commercially available binder.…”

“…To reduce this burden, reduction and stabilization are considered resuscitative measures [43] and fixation of unstable LC fractures has been advocated. In dealing with pelvic trauma, thorough knowledge of pelvic anatomy, fracture mechanism/pattern, and stabilization techniques is necessary [10].…”

“…It is evident that in this group the main, preventable cause of death is hemorrhage and late complications of shock constituted the principal cause of death (32). Thus, the control of bleeding by any means necessary, appears to be the basic and most important element of the management in theese patients (33,34). Trauma surgeon should be aware that hemorrhage is not always a result of extensive organ or large vessel injuries (35).…”
