Cirrhotic cardiomyopathy: Implications for liver transplantation

Abstract: The majority of patients on a waiting list for liver transplantation have end-stage liver disease. Because of the marked peripheral vasodilatation of end-stage cirrhosis that masks a latent myocardial dysfunction, cardiac abnormalities in the resting state are usually subclinical and escape the attention of physicians. However, when challenged, the systolic and diastolic contractile responses are attenuated. In addition to these contractile abnormalities, morphological changes, such as enlargement or hypertrop… Show more

“…Although cellular and molecular pathogenic mechanisms underlying CCM have been elucidated in animal models, progress in the diagnosis and management of CCM has been hampered by lack of universally accepted diagnostic criteria . At the Montreal 2005 World Congress of Gastroenterology, an expert consensus panel proposed preliminary criteria for diagnosis of CCM (Table a); but progress in modern concepts of ventricular dysfunction, especially over the past decade, has rendered these criteria obsolete.…”

“…Although it is often believed that cirrhotic cardiomyopathy resolves post‐LT, the data, albeit limited, do not support this postulation. In fact, the data suggest that diastolic function may not improve post‐transplant and may actually worsen.…”
