Citalopram and cardiac toxicity

Abstract: A dose-effect relationship for QT prolongation exists across a broad range of citalopram doses, such that caution must be exercised when prescribing high doses or if there are co-existent risk factors for QT effects. The available data illustrate how clinical toxicity data may offer an earlier signal of cardiac effects than ascertained from conventional pharmacovigilance methods.

“…The mechanism of cardiac toxicity is thought to be, in part, mediated by one of its metabolites, didemethylcitalopram (DDCT), and its blockade of the inward rectifier potassium channel [12]. As a consequence of QT prolongation, torsades des pointes has also been described in citalopram overdose [5] and was seen in our patient.…”

“…111 Particular attention should be paid to falls, hyponatremia, and gastrointestinal bleeding, which are associated with SSRIs in general 112,113 and to QTc prolongation with citalopram. 114 Standard principles of conservative prescribing should be applied to minimize adverse drug outcomes. 115 Meta-analyses also suggest that longer antidepressant treatment trials (10-12 weeks) are required in LLD.…”

“…Case reports and case series suggest that certain secondgeneration agents, notably citalopram, venlafaxine, and bupropion, as well as selected antipsychotic medications, might be more hazardous in overdose (11)(12)(13)(14)(15)(16)(17)(18)(19). However, few reports have systematically compared the morbidity and mortality of the array of agents used to treat depression.…”
