The children who, together with their guardians, agreed to participate in the study were submitted to the consecutive use of both the traditional and the pre-lubricated catheter, for one month each. During that period, the children completed the data collection instruments and were followed by the researchers once a week through home visits and hospital consultations, including quantitative and qualitative urine cultures that were performed every other week. Data analysis was performed using Wilcoxon and Kaplan-Meier tests. Eleven children completed the study. Statistical significant differences were found only for costs (p=0.003), which were higher for prelubricated catheters. KEY WORDS RESUMENEl estudio, del tipo crosover, objetivó comparar el uso de dos catéteres para cateterismo intermitente limpio en niños con urostomías continentes, en lo que se refiere al manejo, complicaciones y costos directos. Cumplidas las exigencias éticas, fue desarrollado en un Hospital Infantil, en São Paulo. Los niños que, juntamente con sus responsables, consintieron en participar de la investigación, fueron sometidos a la utilización consecutiva de catéter tradicional y de catéter lubrificado, durante un mes cada uno. En ese período, las niños llenaron los instrumentos de recolección de datos y fueron acompañados por las investigadoras semanalmente, por medio de visitas domiciliares y consultas hospitalarias, que incluyeron a recolección quincenal de culturas cualitativas y cuantitativas de orina. Los datos fueron analizados utilizando las pruebas de Wilcoxon y Kaplan Meyer. Once niños completaron el estudio. Los resultados mostraron diferencias estadística-mente significativas apenas en lo que se refiere a costos directos (p=0,003), superiores para el catéter lubrificado. DESCRIPTORESEstomas quirúrgicos. Introduction: The aim of the present study is to compare a hydrophilic catheter to the standard polyvinyl chloride catheter with regard to bacteriological safety and overall comfort in patients undergoing intravesical immuno- or chemotherapy for bladder cancer. Materials and Methods: One hundred patients (80 males, 20 females; median age 65.8 years, range 48–79 years) eligible for intravesical prophylaxis of superficial bladder cancer recurrences were randomized to receive intravesical therapy using a standard catheter (group A, n = 50) or a hydrophilic catheter (group B, n = 50). Urinalysis and urine culture were performed 2 days after catheterization. Comfort during catheterization was assessed by a 5-point visual analogue scale at the end of the first four instillations. Results: Urinary tract infection (UTI) was detected in 7.4% of catheterizations in group A, whereas it occurred in 3.5% of catheterizations in group B (p < 0.01). Escherichia coli was the most frequent pathogen regardless of the device used. At the end of each of the first four instillations, the mean score for discomfort was significantly higher in group A than in group B (p < 0.001), although catheterization was progressively better tolerated regardless of the device used (both p < 0.005). None of the patients were found to be suffering from orchitis, epididymitis or gross haematuria. Conclusion: Hydrophilic catheters may be used safely and are well tolerated by patients undergoing intravesical immuno- or chemotherapy. The hydrophilic catheter was associated with a significantly lower occurrence of UTI and higher acceptability compared to the standard device. These data should be considered with regard to patient compliance with intravesical therapy.

Clean Intermittent Catheterization in Boys Using the Lofric Catheter

Abstract: In boys the LoFric catheter appears to cause less trauma. Although it is not reusable and is more expensive than the standard catheter, satisfaction is higher with the LoFric device and for select patients it has significant advantages.
