Clear-Cell Meningioma in a 22-Month-Old Male

Abstract: We report a case of spinal clear-cell meningioma occurring in a 22-month-old male who presented a right limp and then refused to walk. Spinal magnetic resonance imaging demonstrated a large, intradural tumor from T11 to L4, which was totally excised. The patient’s postoperative recovery was uneventful and 5 months after surgery he began walking again. The latest follow-up magnetic resonance imaging of the brain and spine, obtained 42 months after diagnosis, was negative for tumor recurren… Show more

“…Clear cell meningiomas exhibit a predilection for pediatric patients and typically arise in the lumbosacral spine. [3][4][5][6]8,9,11,12,18,21 The molecular underpinnings of CCM tumorigenesis and progression remain poorly defined. Recently, heterozygous loss-of-function mutations in the SWI/SNF chromatin remodeling complex subunit SMARCE1 were found in 4 individuals with familial multiple spinal meningiomas.…”

“…have reported a case showing extension into the intervertebral foramen. [ 27 ] Our patient's MRI image showed anterior scalloping of the vertebral body and posterior thinning of the lamina with the intervertebral foramen extension. Dural tail sign, classically seen in the meningioma on imaging, was absent in our patient.…”

“…Spinal meningiomas typically attach to the dura, and non-dural spinal meningiomas are rare 1 , 7 , 10 , 12) . Eighteen cases of non-dural based spinal meningiomas have been reported to date ( Table 1 ) 2 , 3 , 4 , 6 , 8 , 9 , 11 , 13 , 14 , 16 , 17 , 18 , 19 , 20 , 21 , 22) . Most tumors in these cases occur in the lumbo-sacral region (16 cases), including the thoraco-lumbar junction (two cases) 2 , 3 , 4 , 6 , 8 , 9 , 11 , 13 , 14 , 16 , 17 , 18 , 19 , 20 , 22) .…”

“…Most of histopathologic subtype of non-dural-based spinal meningiomas was clear cell meningioma (15 cases) 2 , 3 , 4 , 6 , 8 , 11 , 13 , 14 , 16 , 18 , 20 , 21 , 22) , whereas the remaining three cases were angiomatous, transitional, and meningotheliomatous meningiomas 9 , 17 , 19) . The clear cell meningiomas were quite predominant, as high as approximately 83.3%.…”
