Palatogenesis is a complex morphogenetic process, disruptions in which result in highly prevalent birth defects in humans. In recent decades, the use of model systems such as genetically-modified mice, mouse palatal organ cultures and primary mouse embryonic palatal mesenchyme (MEPM) cultures has provided significant insight into the molecular and cellular defects underlying cleft palate. However, drawbacks in each of these systems have prevented high-throughput, large-scale studies of palatogenesis in vitro. Here, we report the generation of an immortalized MEPM cell line that maintains the morphology, migration ability, transcript expression and responsiveness to exogenous growth factors of primary MEPM cells, with increased proliferative potential over primary cultures. The immortalization method described in this study will facilitate the generation of palatal mesenchyme cells with an unlimited capacity for expansion from a single genetically-modified mouse embryo and enable mechanistic studies of palatogenesis that have not been possible using primary culture.

Although cleft palate anomaly is frequent, the criterion standards in surgical treatment have not been determined yet. There are a few techniques described for cleft palate repair owing to the limited tissue in the palatal mucosa, the rigid structure of the palatal mucosa, and the limited vascularity of the hard palate. In this study, a novel cleft palate repair technique based on separating the soft palate from the hard palate as a musculomucosal flap and using it as a rotation flap has been described. The operation is evaluated individually for each anomaly because variations occur in the surgical technique according to the extension of the cleft toward the teeth in the palate. This operation was performed on a total of 28 patients (17 girls and 11 boys) aged between 1.5 and 16 years and presented to our clinic. Patients were assessed for speech analysis outcomes, tympanogram values, hearing functions, magnitude of palatal lengthening during the operation, and rate of fistulae. Statistically significant differences in values of the speech analysis and the audiometric assessment were determined between before and 6 months after surgery. Complete recovery of otitis was observed 1 month after surgery without another treatment in 9 (42.8%) of 21 patients who were detected to have serous otitis media preoperatively. Tension-free closure, lower risk of fistula, good restoration of velopharyngeal functions, ability to be performed on all types of cleft palate, ability to provide a good intraoperative exposure, and being a single stage seem to be the most important advantages of this technique.

Introducción: Frente al alto número de pacientes con fisura labial y/o palatina ya existente en la ciudad de Arica y al incremento constante de nuevos casos, en 1987 decidimos abordar el problema desde un punto de vista multidisciplinario e interdisciplinario, integrando además la valiosa ayuda de los padres de los niños fisurados.Objetivo: Mostrar nuestra experiencia y sus resultados parciales en el manejo multidisciplinario del paciente fisurado en la ciudad de Arica entre los años 1987 y 2002.Material y método: Mediante la creación y puesta en práctica del "Programa de Atención del Paciente Fisurado", en el Hospital Juan Noé de Arica entre los años 1987 y 2002, se atienden 59 pacientes que siguen el programa.Resultados: A modo de resultado preliminar se entrega la experiencia de 59 casos que siguieron el protocolo del programa.Conclusión: El trabajo con el paciente fisurado, es un trabajo de largo aliento. Como factores de importancia se destacan el largo tiempo de seguimiento, el factor crecimiento y la observación atenta de la evolución en cada caso en particular. Resulta imprescindible el trabajo en equipo multidisciplinario, evaluando en conjunto cada caso, elaborando un claro y estructurado plan de acción con cada paciente, así como evaluar en forma periódica y en conjunto los resultados parciales.Palabras clave: Fisura labial palatina, Labiopalatina, Otorrinolaringología, Arica.
