Abstract: The emergence of the COVID-19 pandemic in 2019 forced an abrupt shift in the modality through which psychotherapy was delivered and online therapy became the only viable option for clients. Research regarding experiences of online therapy during the early stages of the COVID-19 pandemic is minimal, however, and has largely focussed on therapists' experiences of delivering online treatment, as opposed to clients' expe-van KESSEL et al.

Background During the COVID-19 pandemic, technologies such as videoconferencing were used to deliver mental health appointments remotely online. For many people, this was a change from previous methods of mental healthcare receipt and delivery. We aimed to explore in-depth how practitioners, young people and parents in the UK experienced this transition. Methods We used qualitative methods to collect data, triangulating between free-text online survey data (n = 38), focus groups (n = 5) (3 young adult groups (total n = 11); 2 practitioner groups (total n = 7)), and semi-structured interviews (practitioners n = 8; parents n = 4). Data were analysed using reflexive thematic analysis. Results Participants held mixed views about remote appointments, which were encompassed within the five themes of: home as clinic; disrupted therapeutic relationships; difficulties with engagement; uncontained risk; and scope of care provision. While appointments at home could be regarded as more comfortable, naturalistic and accessible, it was also recognised that remoteness compromised practitioner control with consequences for their ability to monitor patient engagement, manage risk and ensure confidentiality when others were present in the home. This could create an additional burden for parents as they tried to facilitate appointments but felt unsupported in this role. Relatedly, remoteness was seen to hinder interpersonal communication, formation of trust, communication of empathy and opportunities to observe body language, all of which were deemed important to building and maintaining effective therapeutic relationships. Despite this, others thought the anonymity of a remote exchange may allow earlier disclosure. There was disagreement as to whether remote provision narrowed or expanded the scope of practice. Conclusions While some had positive views of remote mental health appointments, others found them challenging. Findings highlight key areas requiring attention and mitigation in future offerings of remote provision, namely: risk management, parental burden, and problematic engagement.

Aims and method A collaborative evaluation of remote consultations in mental health services was undertaken by mental health service providers, experts by experience, academic institutions and a Health Innovation Network in south London, UK. ‘Learning healthcare systems’ thinking was applied. Workstream 1 reviewed international published evidence; workstream 2 synthesised findings from three health provider surveys of the perceptions and experiences of staff, patients and carers; and workstream 3 comprised an electronic survey on local projects. Results Remote consultations can be acceptable to patients and staff. They improve access for some while restricting access for others, with digital exclusion being a key concern. Providing tailored choice is key. Clinical implications The collaboration generated learning to inform choices by healthcare providers to embed or adapt remote delivery. A key output was freely downloadable survey questions for assessing the quantity and quality of appointments undertaken by phone or video or face to face.

La pandemia da Coronavirus (Covid-19) ha richiesto l'adattamento di pazienti e psicoterapeuti a un'evoluzione del tradizionale setting "in presenza" alla videoterapia (VT). La VT ha visto una progressiva diffusione prima come passaggio obbligato e, poi, come nuova modalità alternativa o integrabile. L'obiettivo di questo lavoro è indagare il grado di soddisfazione dei pazienti nel passaggio alla VT, unitamente all'impatto di attitudini/esperienze pregresse e attuali riguardo alla VT stessa. A tal fine, è stato sviluppato un questionario ad hoc, inviato via e-mail a tutti i pazienti seguiti in psicoterapia, tra marzo e giugno 2020, presso il servizio di Psicologia,Psicoterapia e Psichiatria del Centro Medico Santagostino. È stata condotta un'analisi della distribuzione delle risposte nel campione (n = 111) e un modello di regressione multipla per valutare l'effetto di specifiche variabili sul grado di soddisfazione espresso dai pazienti. Le difficoltà nell'uso dei sistemi di video-comunicazione e di postazione predicono una minore soddisfazione per la VT. Nell'esperienza di VT, un senso di maggiore spontaneità con il terapeuta e la percezione di impatto sul modo di comunicare correlano, rispettivamente positivamente enegativamente, con la soddisfazione per la VT. Al fine di una migliore esperienza di VT, proponiamo di considerare diverse variabili contestuali e specifiche che ne caratterizzano il setting.
