Background: Climate change influences patterns of human mobility and health outcomes. While much of the climate change and migration discourse is invested in quantitative predictions and debates about whether migration is adaptive or maladaptive, less attention has been paid to the voices of the people moving in the context of climate change with a focus on their health and wellbeing. This qualitative research aims to amplify the voices of migrants themselves to add nuance to dominant migration narratives and to shed light on the real-life challenges migrants face in meeting their health needs in the context of climate change. Methods: We conducted 58 semi-structured in-depth interviews with migrants purposefully selected for having moved from rural Bhola, southern Bangladesh to an urban slum in Dhaka, Bangladesh. Transcripts were analysed using thematic analysis under the philosophical underpinnings of phenomenology. Coding was conducted using NVivo Pro 12. Findings: We identified two overarching themes in the thematic analysis: Firstly, we identified the theme “A risk exchange: Exchanging climate change and health risks at origin and destination”. Rather than describing a “net positive” or “net negative” outcome in terms of migration in the context of climate change, migrants described an exchange of hazards, exposures, and vulnerabilities at origin with those at destination, which challenged their capacity to adapt. This theme included several sub-themes—income and employment factors, changing food environment, shelter and water sanitation and hygiene (WaSH) conditions, and social capital. The second overarching theme was “A changing health and healthcare environment”. This theme also included several sub-themes—changing physical and mental health status and a changing healthcare environment encompassing quality of care and barriers to accessing healthcare. Migrants described physical and mental health concerns and connected these experiences with their new environment. These two overarching themes were prevalent across the dataset, although each participant experienced and expressed them uniquely. Conclusion: Migrants who move in the context of climate change face a range of diverse health risks at the origin, en route, and at the destination. Migrating individuals, households, and communities undertake a risk exchange when they decide to move, which has diverse positive and negative consequences for their health and wellbeing. Along with changing health determinants is a changing healthcare environment where migrants face different choices, barriers, and quality of care. A more migrant-centric perspective as described in this paper could strengthen migration, climate, and health governance. Policymakers, urban planners, city corporations, and health practitioners should integrate the risk exchange into practice and policies.

Desplazados climáticos: Análisis de la interacción entre cambio climático, vulnerabilidad social y exposición

Existe una estrecha interrelación entre el hombre y el medio en el que éste vive. Los humanos han sido moldeados por el ambiente, así como el ambiente ha sido moldeado por los humanos. A partir de los comienzos del siglo XIX con el inicio de las actividades industriales, las emisiones de gases de efecto invernadero (GEIs), especialmente de dióxido de carbono, resultantes de la combustión de combustibles fósiles, indujeron cambios en los patrones climáticos, sobre todo a través de un aumento medio de la temperatura mundial con los subsecuentes impactos sobre la frecuencia y cantidad de precipitaciones, entre otros. Estos cambios en el estado del clima son lo que se conoce como cambio climático, cuyos principales impactos se observan en la agricultura, ya que esta actividad es extremadamente vulnerable a este fenómeno. Esto hace pensar que el cambio climático impactará de manera aún más desfavorable a los países en desarrollo en los que la agricultura representa un mayor porcentaje de su PBI y de población empleada y en donde los agricultores carecen de acceso a técnicas y recursos para adaptarse, lo que representa una amenaza a la seguridad alimentaria. El estudio de esta problemática debe ser abordado desde una perspectiva integral, sin perder de vista el contexto en el que se manifiestan las alteraciones producto del cambio climático, ya que la manera en que las alteraciones físicas impacten sobre una sociedad, dependerá de la situación social, económica y política previa a la ocurrencia de ese impacto, es decir, de su grado de vulnerabilidad. En aquellas regiones en las que las alteraciones en los patrones climáticos se manifiesten con mayor intensidad, adaptarse las nuevas condiciones se convierte en el principal reto de los agricultores, especialmente aquellos que se encuentran en países en desarrollo, donde la vulnerabilidad socioeconómica y política es mayor. Quienes no pueden adaptarse a los cambios para poder subsistir, se desplazan dentro o fuera de las fronteras nacionales. Observando el caso particular de Siria, entre los años 2007 y 2010 se registró una fuerte sequía vinculada los efectos del cambio climático que provocó reducciones en los rendimientos agrícolas y mortandad del ganado. Esto se conjugó con problemáticas sociales, económicos y políticos, lo que impulsó el desplazamiento de los agricultores del campo a las ciudades. Por su parte, en Bangladesh, se sucedieron dos ciclones, Sidr en 2007 y Aila en 2009 que causaron graves impactos y pérdidas agrícolas. Aquí, los pobres rurales se vuelven incapaces de satisfacer sus necesidades por lo que deciden desplazarse, principalmente de forma interna. La presente tesis intenta explicar la relación existente entre cambio climático; condiciones socio-económicas y políticas preexistentes y desplazamientos dentro y fuera de las fronteras en el caso de las poblaciones sirias y bangladesí, poniendo el foco en la percepción social del riesgo como resultado de la interacción de la vulnerabilidad, la exposición y la ocurrencia de fenómenos climáticos extremos.
