Clinical and biochemical comparison of clorazepate and diazepam

Abstract: SYSNOPSISClorazepate and diazepam were compared with respect to clinical effectiveness and concentrations of benzodiazepine compounds in plasma in 15 severely anxious outpatients. Each patient was studied in a double blind trial incorporating two-week periods of the two drugs and of placebo. The doses were almost equimolar—5 mg diazepam or 7·5 mg clorazepate three times daily. Clinical progress was assessed by visual analogue scales and by the Symptom Rating Test. Psychopathology scores were highest at the end… Show more

“…In a study of depressed patients and normals, SRT scores discrimi nated patients and agreed with the results of psychiatrists' ratings using the Hamilton De pression Rating Scale [13]. SRT scores were also correlated with the concentration of benzodiazepam measured in the blood of anxious patients [14], Higher levels of dis tress were associated with lower concentra tions.…”

“…Later analysis showed that the plasma nordiazepam levels in our patient had been persistently about 10 times higher than the levels expected with regular clorazepate therapy (Robins, Curry & Whelpton, 1974) yet apart from the mild side effects of drowsiness, nystagmus and ataxia, she had remained well. The 42 h nordiazepam level was the highest recorded though the patient was apparently fully recovered by this time.…”

“…Clorazepate was clinically introduced in 1972 and is being used in several countries (Charalampous et al 1973, Holmberg and Livstedt 1972, Robin et al 1974, Percy 1975. In as much as the development of dependency and delirium symptoms has been reported previously in connection with benzodiazepines (Barten 1965, Peters, Boeters 1970, Hanna 1972, Misra 1975 it is not surprising to find that clorazepate also causes these symptoms.…”
