Clinical and histological sequelae of Roux-en-Y diversion

Abstract: Nine post-Polya gastrectomy patients with symptoms of reflux gastritis were studied immediately before and 6 months after Roux-en-Y diversion with a 60 cm jejunal loop. Endoscopy, histology, HIDA radionuclide studies and acid secretory studies were performed to determine the effect of Roux-en-Y diversion. Symptomatic results were pleasing in that all patients were graded as Visick I or II. There was a statistically significant improvement in endoscopic evidence of mucosal damage (median score of 6 reduced to m… Show more

“…The median follow-up was 12 years (Åhsberg, Hammar, Stae ¨l von Holstein, in press). This is in contrast to the results in previously published studies where improvement was seen for some precancerous changes [67][68][69], though most histologic changes remained unchanged [68 -70]. The follow-up time in those studies varied from 6 months to around 4 years.…”

“…The mean(s.e.m.) DeMeester score was 31.1 (4)(5)(6)(7)(8)(9). The percentage of time at which absorbance was greater than 0.15 in the supine period did not differ from that in controls (Table I ) .…”

“…Bile reflux is well recognised as a factor associated with the induction of acute pancreatitis, particularly when associated with sepsis [12, 13]. Reflux of both bile and intestinal contents is prevented by the use of a long isolated Roux loop reconstruction [14, 15]. In 1994 Kingsnorth [16] reported no pancreatic leaks in a series of 52 patients undergoing PD when performing a duct-to-mucosa anastomosis with this reconstructive technique.…”
