Clinical and Incidental Perineural Invasion of Cutaneous Squamous Cell Carcinoma

Abstract: IMPORTANCE Perineural invasion (PNI) in cutaneous squamous cell carcinoma (CSCC) has been associated with an increased risk of poor outcomes. Patients with PNI may present with clinical symptoms and/or radiologic evidence of PNI (clinical PNI [CPNI]), yet most patients are asymptomatic and PNI is often found on histologic examination (incidental PNI [IPNI]). Evidence-based estimates of the risks of disease-related outcomes comparing IPNI and CPNI are limited in the dermatology literature.OBJECTIVES To review a… Show more

“…42 The recurrence rate for cSCC at 25.5% in our study is in accordance with former published studies, which report rates from 17% to 37%. 46,47 Nevertheless, all cutaneous carcinomas of the pinna are reported to be in a hazardous location for neoplasia and are therefore labeled high-risk carcinoma with an increased risk of recurrence and morbidity. 33,47 This knowledge from multiple earlier publications is consistent with the data of this study.…”

“…29 Another study noted that patients with clinical PNI, as evident on physical examination or imaging, had far worse outcomes than patients with incidental PNI that was found on histology, with clinical PNI patients having over twice the rate of recurrence and four times the rate of diseasespecific death. 30 Studies have found that PNI in BCC is found in larger, more aggressive tumors and increase the risk of 5-year recurrence. 31,32 Overall, however, there have been few studies examining the effect of PNI in BCC specifically, as most studies have combined BCC and cSCC.…”

“…An excellent recent review on PNI is Karia et al 28 on PORT in PNI is Han et al 29 A reasonable working model is based on these three scenarios. Incidental PNI is when there have been no symptoms of PNI and PNI is reported on the histopathology form.…”
