Dental caries is the most common non-communicable disease in children with significant aesthetic, functional, and quality of life deterioration. Depending on the depth, two approaches may be considered in primary dentition: vital pulp therapy (VPT) or non-vital therapy (NPT). This umbrella review aimed to critically assess the available systematic reviews (SRs) on VPT and NPT. An electronic database search was conducted (PubMed, Embase, Scopus, Cochrane, Web of Science, and LILACS) until June 2021. The Risk of Bias (RoB) of SRs was analyzed using the Measurement Tool to Assess SRs criteria 2 (AMSTAR2). From 272 entries, 33 SRs were included. Regarding the methodological quality, three studies were critically low, nine low, seventeen moderate, and six were rated as high quality. The quality of evidence produced by the available SRs was moderate. Future high standard SRs and well-designed clinical trials are warranted to better elucidate the clinical protocols and outcomes of VPT and NPT.

Aim The aim of this clinical trial is to compare the clinical and radiographic success of Aloe vera and formocresol (FC) as a pulpotomy medicament in primary molars. Materials and methods A total of 72 asymptomatic or symptomatic vital primary molars were selected and were assigned to two groups: group II: Buckley's FC, group II: Aloe vera gel. The clinical and radiographic success were evaluated using Zurn and Seale criteria. The data were statistically analyzed using the Chi-square test. Results At three and 6 months follow-up, the clinical success rate of Aloe vera and FC was equally effective. Aloe vera showed a higher radiographic success rate compared to FC ( p > 0.05). At 6 months follow-up, FC showed a higher radiographic success rate compared to Aloe vera ( p > 0.05). Conclusion The clinical and radiographic success of both groups were equally effective at the end of 6 months follow-up. Aloe vera can be considered as an alternative pulpotomy medicament to FC. Objective: This systematic review and meta-analysis aimed to evaluate the overall, clinical, and radiographical success rates of alternative pulpotomy medicaments in primary teeth. Methods: A systematic search of five databases was conducted following the PRISMA guidelines. Prospective clinical trials utilising alternative pulpotomy medicaments in children were included. The outcome measures were overall, clinical, and radiographic success, expressed in percentages and converted to odds ratios (OR). Fifteen articles were included in the meta-analysis. Results: combined OR for overall, clinical, and radiographic success was 0.55 [95% CI: 0.12-2.41; P=0.42; I 2 =76%], 1.03 [95% CI: 0.57-1.86; P= 0.92; I 2 =0%] and 0.84 [95% CI: 0.54-1.47; P=0.66; I 2 =34%], respectively. The results suggest an inconclusive outcome in the success rate of alternative medicaments. Conclusions: There is insufficient evidence to support the efficacy of alternative pulpotomy medicaments for use in primary teeth. Further robust studies are required before such alternative medicaments should be used in clinical practice.
