Clinical and radiological long-term outcome after posterior cruciate ligament reconstruction and nonanatomical popliteus bypass

Abstract: The challenging group of patients treated by PCL reconstruction and popliteus bypass according to Mueller et al. or popliteus refixation showed only moderate clinical and radiological long-term outcome without statistical difference, even if patient age at surgery and the long-term follow-up is acknowledged. Anatomical posterolateral corner reconstruction techniques should be preferred.

“…Correlation with Tegner activity scores was 0.56 in one study, suggesting measurement of a separate activity-related construct 28. In other studies, postoperative IKDC scores corresponded with Lysholm, Cincinnati, KOOS, and Orthopädische Arbeitsgruppe Knie (OAK) scores 9–12 15 16 19 21 24 25 27–49. However, these studies did not detail individual patient scores, and thus, correlations with the IKDC could not be quantified.…”

“…Biomechanical data argue in favour of anatomic ligament replication to restore optimal stability [5, 6, 9, 10, 22-25, 28, 32]. Clinical assessments following transtibial and tibial inlay techniques yield satisfactory and promising results [1,8,16,18,21,30,35]. Since several co-variables affect the long-term outcome of PCL reconstruction, the verification of predictive values that assure high patient satisfaction and pre-injury knee function hardly seems to be realistic.…”

“…To restore normal knee joint function and stability and to prevent premature joint degeneration, reconstruction of isolated posterior cruciate ligament (PCL) ruptures has been recommended if both subjective and objective instability are present [1,5,6,9,17,21,31]. Nevertheless, the ideal management protocol is controversially discussed [18,29,33,35].…”

“…However, both methods do not yield significantly better treatment outcome compared with the arthroscopic single-bundle method. 29,35,36 More anatomical PCL reconstruction techniques have been advocated to achieve better surgical results, 37,38 but the optimal surgical technique of PCL reconstruction remains to be established.…”
