Clinical and Research MRI Techniques for Assessing Spinal Cord Integrity in Degenerative Cervical Myelopathy—A Scoping Review

Abstract: Background: Degenerative cervical myelopathy (DCM) manifests as the primary cause of spinal cord dysfunction and is non-traumatic, chronic and progressive in nature. Decompressive surgery is typically utilised to halt further disability and neurological dysfunction. The limitations of current diagnostic options surrounding assessment and prognostic potential render DCM still largely a clinical diagnosis. Aims: To outline the limitations of current diagnostic techniques, present evidence behind novel quantitati… Show more

“…22 Alruwaili et al, studied TB spine surgery in the dorsal spine by transthoracic approach and found improvement in neurology postoperatively. 23 In the lumbar region, 3 patients required a retroperitoneal approach with decompression and fusion with cage and bone graft but in 10 patients' posterior decompression, debridement, and fusion with trans-pedicle screw fixation were done 16,20 (Table 3).…”

“…These goals can be achieved by the anterior approach, anterolateral approach, posterolateral approach, or the posterior approach to the spine. 16 In the anterior surgical approach, transthoracic thoracotomy is done which is followed by debridement of debris and necrotic material and fusion with a mesh cage. In the posterior or posterolateral approach, laminectomy or transpedicular decompression is done which is followed by transpedicular screw fixation and fusion with bone graft.…”

“…In addition to conventional sequences, numerous sophisticated neuroimaging methods, such as diffusion tensor imaging (DTI), magnetization transfer (MT), myelin water fraction (MWF), and magnetic resonance spectroscopy (MRS), offer in-depth quantitative information on microstructural alterations in the spinal cord, providing an enhanced understanding of the disease pathophysiology 6,7 . Several studies have explored the application of these techniques in patients with DCM to assess spinal cord injury severity and predict the outcome of surgical interventions 2,6,[8][9][10][11] . However, unfortunately, the ability to monitor DCM development and predict the potential for recovery is still limited, even with advanced neuroimaging techniques.…”

“… 7 , 15 , 16 Hence, DTI should not merely be viewed as an auxiliary diagnostic assessment, but rather as an essential instrument in detecting DCM and a precursor in determining those patients who are most appropriate for surgical intervention. 17 Although numerous investigations have explored the application of DTI in the context of DCM, the majority of these studies are characterized by a limited sample size 14 and DTI has not been routinely incorporated into the clinical management of DCM patients. Past literature reviews have attempted to assess the relevance of DTI metrics in DCM patient populations; however, these were largely scoping or quantitative reviews, making their conclusions predominantly qualitative.…”
