Clinical and videofluoroscopic outcomes of laparoscopic treatment for sliding hiatal hernia and associated gastroesophageal reflux in brachycephalic dogs

Abstract: Objective: To describe a laparoscopic technique for treatment of sliding hiatal hernia (SHH) and associated gastroesophageal reflux (GER) in brachycephalic dogs and document clinical and videofluoroscopic outcomes postoperatively.Study design: Prospective clinical trial. Animals: Eighteen client-owned dogs. Methods: A three-port laparoscopic approach was used. Intracorporeal suturing was used for hiatal plication and esophagopexy, and left-sided laparoscopic or laparoscopic-assisted gastropexy was performed. A … Show more

“…Duration of time spent upright in a Bailey chair to facilitate gravityassisted feeding and treatment modifications can be advised for dogs with megaesophagus (41,166). Follow-up fluoroscopy can also assess treatment responses or outcomes in dogs or humans that undergo hiatal hernia surgery or achalasia interventions (43,167,168).…”

“…The clinical applications of swallowing fluoroscopy extend beyond diagnosis. It can be utilized to assess outcomes of medical (sildenafil, botulinum toxin, pneumatic dilation) (167,216) or surgical management (myotomy with fundoplication) (167) of esophageal achalasia, prokinetic and proton pump inhibition or surgical treatment of GER or hiatal herniation (43,168), and laser or surgical myectomy for cricopharyngeus muscle achalasia (39).…”

“…Procedures where small imprecisions in technique carries risk for high morbidity, such as cholecystectomy and adrenalectomy, have become established advanced MIS procedures. More recently, procedures of high technical challenge including intracorporeal suturing have been described, 5,6 and this development is likely to continue. These challenging procedures require a high level of surgical skills, and currently we have no mechanism for determining if a surgeon is competent to perform them or not, other than crossing our fingers and hoping for the best.…”
