“…I found the solution in the so‐called “Y” system, which I presented for the first time in 1979 (3). This system completely revolutionized the philosophy of the connection (4–10). In fact, with the wearable‐bag system, the first step after the most possibly contaminating act, that is, the disconnection of the bag with the spent dialysate and the connection of the bag with the fresh solution, was the filling of the abdominal cavity.…”

“…This system was introduced into clinical practice in 1979 (2, 3) and the first clinical results were published in 1980 (4) and 1983 (5). Since then, the efficacy of the system has been largely confirmed by a great number of uncontrolled but substantial trials (6)(7)(8)(9)(10)(11)(12)(13) and also by some meticulous prospective controlled trials (14). During the last few years, experience has grown steadily and rapidly chiefly in Italy where more than 95% of CAPD patients are on this system.…”

“…It is important for us all to remember that many (or even most) major advances in PD devices have not come through industry but through the innovation and ingenuity of healthcare workers and researchers. The Y-set, 50 53,51,52,54 flush before fill connectology 55,56 and icodextrin 57 59 are all examples of ‘game changers’ brought to light from independent scientists and clinicians, before being handed to industry for large-scale manufacture. It is timely to remind ourselves that improving PD devices is far from being the sole provenance of industry R&D, which at this current time is more focused on quality manufacturing, rather than basic product innovation.…”
