Clinical Application of Artificial Dermis and Autologous Skin in Repairing Skin and Soft Tissue Defects of Hands and Feet with Bone Exposure Injuries

Abstract: Patients with skin and soft tissue defects are very common. Mild trauma often causes mild skin damage, while severe injuries are often accompanied by bone and tendon exposure, which brings great pain to patients. For the defect of skin and soft tissue, the traditional treatment methods are mostly medium or full-thickness skin or skin flap transplantation. These methods are effective in wound repair, but there are still many problems. In recent years, with the improvement of tissue engineering technology, the u… Show more

“…Various soft tissue defects can be satisfactorily treated using artificial dermis. [2][3][4] However, it takes 2-3 weeks from applying artificial dermis to the wounds until sufficient granulation tissue is ready for skin grafting. This delay results in various complications, such as surgical site infections, particularly in the treatment of chronic skin ulcers.…”

“…Simultaneously, the induced membrane capsule not only distributes small blood vessels in the long axis direction but can also secrete vascular endothelial growth factor, transforming growth factor-beta 1, and bone morphogenetic protein-2 to promote local blood vessel formation. [ 12 , 13 ] However, no cases have been reported of direct coating of the bone with antibiotic-containing bone cement to form fresh induced membranes. In our patient, after the drilling, bone cement covered the bone surface.…”

“…Artificial dermis combined with VSD has also been used to repair the donor site of the great toe, but it is only suitable for repairing small area defects, not large ones 9 . Li et al used artificial dermis combined with autogenous blade thick skin to repair hand and foot skin and soft tissue, but the artificial dermis is susceptible to infection, which is not conducive to wound recovery, and there are still many limitations 10 . Many scholars further use the pedicled island flap to repair the toe donor area.…”

According to reports, Özay Özkaya et al achieved satisfactory results in repairing the defect of the big toe with the first dorsal metatarsal artery island flap.…”
