Objective. The purpose of this study was to assess vestibular findings and clinical history in a large cohort of patients affected by Ménière's disease. Methods. We retrospectively analysed 511 adult patients fulfilling criteria for definite unilateral Ménière's disease according to Barany Society. Thorough clinical history, audiometric exam, central nervous system MRI, quantification of serum autoantibodies and complete vestibular function test were performed. Results. Mean age at clinical record was 55.4 years, while age at onset of the first vertigo attack was 47.4 ± 14.3 years. Ménière's disease overlapped with migraine in 43.4% of patients. In 31.7% of cases, positivity was found for at least one autoantibody. Forty-nine patients (9.6%) had family history for Ménière's disease. Bedside examination resulted in 14.7% positivity for video head impulse test, 58.9% for skull vibration-induced nystagmus, 38.7% for the positional test and 23.1% for the post head shaking test. Complete negative examination was reported in 115 cases. Conclusions. Ménière's disease was seen to present a characteristic phenotypic pattern in our cohort, confirming the crucial role of thorough anamnesis and bedside examination in diagnosis.

Introduction. Migraine vertigo (MV) and Ménière's disease (MD) share several signs and symptoms such as tinnitus, fullness, photophobia, phonophobia, headache and vertigo spells lasting hours. Objective. The aim of the present study was to prospectively observe patients with MV with a sporadic audiological symptom evaluated with clinical examination, Vestibular Evoked Myogenic Potentials (VEMPs), ECochG and v-HIT. Results. The finding of VEMP asymmetry, according to our cut-off of 33% of difference between sides, resulted in 20 cases, of which 6 had asymmetry of both c-VEMPs and o-VEMPS, all with development of fluctuating hearing during follow-up. ECochG was positive for endolymphatic hydrops in 12 patients. Conclusions. The evolution of MV may have a variable course in which some patients may develop symptoms typical of MD. The two diseases may be contextually present at the same time configuring an overlapping syndrome, and asymmetric VEMPs might predict development of fluctuating hearing.

Background: Meniere's disease is idiopathic Endolymphatic Hydrops. Vestibular evoked myogenic potential is a neurophysiological assessment technique that is used determine the function of the otolith organs of the inner ear. Using vestibular evoked myogenic potential tests may identify the different forms of the Endolymphatic Hydrops. Aim of Study: The aim of this study is to explore the role of vestibular evoked myogenic potentials (VEMP) in diagnosis of the different kinds of Meniere's disease (MD). Patients and Methods: Three groups of patients that were selected according to the diagnosis, definitive MD (DMD), cochlear MD (CMD) and recurrent vestibulopathy (RV) group. Basic audiological evaluation was carried out. It included Pure tone audiometry, speech audiometry, acoustic immittance testing and electrocochleography. Otolith function assessment was carried out in the form of: Cervical VEMP (c-VEMP) and ocular VEMP (o-VEMP). Vestibular assessment was carried out: The test protocol included spontaneous nystagmus, gaze test, oculomotor tests, positioning test (Dixhallpike), positional nystagmus, and caloric testing. The three groups were compared for all demographic variables as well as clinic-pathologic variables, and tests results. Results: The overall sample was 40 cases; 13 with DMD, 10 with CMD and 17 with RV. The mean age is variable between groups (p-value <0.001) with the highest in the DMD group (38.3±7.4 years) and the lowest in the CMD group (25.2±4.6 years). The male to female ratio is variable between groups of the study (p-value=0.03) with the highest ratio 9:4 in the DMD group. There was a significant difference between groups (p<0.001) regarding dizziness. Asymmetric response of c-VEMP was seen in nine (9/13) (69.2%) of DMD cases, (14/17) (82.4%) of RV cases and only in three (3/10) (30%) of CMD cases, (p-value <0.001). Low amplitude of o-VEMP was seen in (11/13) (84.6%) of DMD cases, (9/10) (90%) of CMD cases, (15/17) (88.2%) of RV cases, and only in two (25%) of normal cases, (p-value=0.002).
