Clinical application of cVEMPs and oVEMPs in patients affected by Ménière’s disease, vestibular neuritis and benign paroxysmal positional vertigo: a systematic review

Abstract: Vestibular evoked myogenic potentials (VEMPs) are increasingly used for different pathologies with new clinical insights. Although the study of otolithic function selectively in both its saccular (cervical VEMPs) and utricular (ocular VEMPs) parts does not represent a recent achievement, the clinical utility of this tool is still emerging. The aim of the present report is to define advances in application of VEMPs in diagnosis and clinical study of vestibular neuritis, Ménière's disease and benign paroxysmal p… Show more

“…Further studies can assess if a dissociation between the 2 tests may be a peculiar characteristic of MD. Moreover, SVIN mainly detect a deficit of the lateral canal function; other tests, ocular and cervical VEMPs, are promising in studying macular function, above all when performed with sounds at different frequencies and studying the tuning curve of responses, thus allowing a mapping of all parts of the inner ear 28,29 . Some weak points in our investigation should be underlined.…”

“…As is known, c-VEMPs is the first instrumental test that shows some alteration in MD due to early involvement of the saccule by endolymphatic hydrops with high sensitivity of the macula to low frequency 17,18 .…”

“…Therefore, the air conducted VEMP could be a relevant addition to the diagnosis workup of patients with possible MD. Scarpa et al, in their study about clinical applications of cVEMPs and oVEMPs in patients affected by Meniere's disease; lower response rate, smaller amplitude, and higher threshold of the o-VEMP indicated the pathological disease evidence [27].…”
