Clinical assessment of acute lateral ankle sprain injuries (ROAST): 2019 consensus statement and recommendations of the International Ankle Consortium

Abstract: Lateral ankle sprain injury is the most common musculoskeletal injury incurred by individuals who participate in sports and recreational physical activities. Following initial injury, a high proportion of individuals develop long-term injury-associated symptoms and chronic ankle instability. The development of chronic ankle instability is consequent on the interaction of mechanical and sensorimotor insufficiencies/impairments that manifest following acute lateral ankle sprain injury. To reduce the propensity f… Show more

“…At this point, the clinician approaches the patient and performs a clinical examination, including range of motion, strength, balance, and functional activities, as dictated by usual care and current evidence. 50,51 If, based on the clinical examination and study protocol, the patient may benefit from a joint-mobilization treatment protocol, the clinician describes the study and asks if the patient would like to participate. Figure 3 is a screenshot of the clinician's view in the EMR that shows a summary of the screening and enrollment status of patients.…”

“…Most recently, a third statement from the IAC was published in 2018 in BJSM to present an evidence-informed consensus on the clinical assessment of patients with acute LASs. 7 Using a Delphi method, the executive committee and selected clinicians from around the world arrived at recommendations for undertaking a structured clinical assessment of acute lateral ankle sprains. This consensus yielded the Rehabilitation-Oriented ASessmenT (ROAST) statement, which guides clinicians in objectively identifying mechanical and sensorimotor impairments after acute LAS that are associated with CAI.…”

“…This modified Delphi process informed the development of a consensus statement on a structured clinical assessment for acute lateral ankle sprain injuries. Our Delphi process has resulted in the development of a pragmatic minimum standard clinical diagnostic assessment, as well as an initial rehabilitation-oriented assessment 16. The primary focus of the International Ankle Consortium Rehabilitation-Oriented ASsessmenT (ROAST) is to help clinicians identify the primary mechanical and/or sensorimotor impairments that have been commonly observed in individuals with chronic ankle instability.…”
