Clinical Characteristics and Outcomes of Patients With Recurrence 5 Years After Nephrectomy for Localized Renal Cell Carcinoma

Abstract: Data suggest that patients with cancer recurrence 5 years after nephrectomy are at favorable risk and have long-term median survival. A favorable Memorial Sloan-Kettering Cancer Center risk score and absent symptoms related to metastasis are associated with longer survival in these patients.

“…5 Studies have characterized patients with late relapse, occurring after a disease-free interval >5 years, in terms of patient and tumor prognostic features to adjust surveillance protocols and increase the length of surveillance in patients with an increased propensity to relapse after 5 years. 6,7 A 5-year disease-free interval is most commonly used to define the late-relapse population and has been shown to be the most useful interval length in clinical practice. 7 The prognosis and response to targeted therapies of patients with late relapse are less studied.…”

“…After an initial 5-year postoperative disease free interval the late recurrence rate was reported as 6-20% (8,13). The most recurrences were distant rather than local.…”

“…Most recurrences are distant rather than local. [82][83][84] The largest study evaluating relapse after 5 years following nephrectomy demonstrated lymphovascular invasion, Furhman grade 3 or 4, and pathologic tumour stage >pT1 as independent predictors of late recurrence. In addition, late recurrence was approximately 2.6%, 5%, 9%, 10%, 11% and 22% for T1a, T1b, T2a, T2b, T3a and T3b respectively.…”
