Clinical Characteristics of Itch in Renal Transplant Recipients

Abstract: Background: Chronic itch is the most common symptom in dermatology. End-stage renal disease-associated chronic itch (ESRDCI) is a common burden affecting up to 35% of patients treated with hemodialysis. Kidney transplant (KTx) is believed to be the best renal replacement therapy leading to the elimination of ESRDCI. The study was undertaken to characterize and assess the prevalence of itch among patients after renal transplantation. Methods: Between October 2019 and January 2020, we analyzed the data of 197 pat… Show more

“…Standardized questionnaires and scales were completed by patients in order to assess the itch intensity [Numerical Rating Scale (three days, 24hours) (NRS; 0–10 points)] and the Four-item Itch Questionnaire [ 11 , 12 , 13 ] (4IIQ; 3–19 points). NRS cut-off points are as follows: 1– < 3 points represent a mild itch, 3–7 points a moderate itch, ≥7–9 points a severe itch, and ≥9 points very severe itch [ 14 ].…”

“…Following RTx, still no guarantee can be made on the disappearance of CKD-aI. Nevertheless, it bothers significantly lower fraction of adult patients than previously [ 26 , 27 , 28 ]. Based on our personal experience, children initially suffering from CKD-aI also benefit from RTx, with the majority of young recipients finally free from this symptom (unpublished data).…”

“…Based on recent data, it affects 42–44% of adults undergoing haemodialysis (HD) [ 22 , 23 ], while in patients undergoing peritoneal dialysis (PD) the percentages exceed 60% [ 24 , 25 ]. Patients who underwent successful renal transplantation (RTx) still complain of CI in up to 32% cases, albeit the percentages are lower than during their previous HD sessions [ 26 , 27 , 28 ]. However, CKD-aI may also bother CKD patients not yet on dialysis, including those with earlier stages of the disease [ 23 , 29 , 30 ].…”

“…A total of 42 (21.3%) renal transplant recipients reported itch, of which 22 (52.4%) cases reported the appearance of itch following transplantation. However, those with itching after renal transplant responded well to HD [ 81 ].…”
