Clinical, double blind, randomized controlled trial of experimental adhesive protocols in caries-affected dentin

Abstract: Adhesive protocols may influence the success of resin composite restorations in CAD; this is important because failure can lead to caries, re-incidence, and/or clinical re-work.

“…The conservation of dental tissue, either intact or subject to remineralization, has been basing the concept of minimally invasive dentistry ( Ismail et al, 2013 , Philip, 2019 ). In this way, clinical interventions should be limited to the removal of friable enamel and softened dentin (caries-infected dentin), preserving enamel and partially demineralized dentin (caries-affected dentin) regions ( Pintado-Palomino et al, 2019 ).…”

“…Encouraging minimal invasive dentistry ( Mm et al, 2014 ), mainly for dental caries treatment ( Thompson et al, 2008 , Bjørndal, 2018 ), has led to the development of conservative strategies ( McComb, 2000 , Pintado-Palomino et al, 2019 ) that involve the preservation of the affected dentin for lesion treatment ( Statement, 2017 ). In this regard, the development of bioactive restorative materials has become a necessity and it has been the subject of studies ( Abuna et al, 2016 , Amrollahi et al, 2016 , Braga, 2019 , Sauro and Pashley, 2016 ).…”

“…Meanwhile, there is evidence indicating that structural changes in CAD are reversible (Meraji et al, 2018 ). Consequently, minimally invasive restorative dentistry is recommended to preserve the CAD to protect the tooth structure and vitality of the pulp in direct and indirect restorations (Elsharkawy, 2018 ; Pintado‐Palomino et al, 2019 ). However, because of the chemical and mechanical alterations in CAD, such as a thicker and irregular smear layer with fibril‐like structures, and the presence of narrow and occlude dentinal tubules (Meraji et al, 2018 ; Pintado‐Palomino et al, 2019 ; Taniguchi et al, 2009 ), bonding to CAD is challenging.…”

“…7,8 Using this strategy, the soft and firm dentin are removed, and the demineralized hard dentin is maintained because it is a remineralizable tissue. 9 Contaminated or demineralized dentin close to the pulp floor should be preserved to avoid accidental pulp exposure. 10 Partial removal of carious tissue is a conservative approach that preserves the underlying remineralized dentin.…”
