A long-acting local anaesthetic is desirable in analgesic blocks. Even in operations in which post-operative pain may be expected to be particularly troublesome, e. g. in haemorrhoid operations, anaesthesia with a long-acting local anaesthetic must be considered of invaluable help. Many attempts have been made to produce local anaesthetics with long duration. However, owing to serious side effects, inter alia, irreversible nerve damage, none of the agents previously tested have come into general use. LAC-43 (Marcahem), an agent synthetised at AB Bofors Nobelkrut, Bofors, is a new long-acting local anaesthetic. Chemically, LAC-43 is 1-n-butyl-DL-piperidine-2-carboxylic acid-2,6dimethylanilide hydrochloride and the formula is given in Fig. 1. The toxicity and effect of the drug have been carefully investigated in tests on animals (HENN and BRATTSAND ( 1966)5). Thorough examinations of LAC-43, e.g. intravenous toxicity in man ( JORFELDT et al.
