Purpose: Experimental animal models of myopia demonstrate that higher melatonin (Mel) and lower dopamine (DA) concentrations actively promote axial elongation. This study explored the association between myopia and serum concentrations of DA and Mel in humans. Methods: Morning serum concentrations of DA and Mel were measured by solid phase extraction-liquid chromatography-tandem mass spectrometry from 54 participants (age 19.1 AE 0.81 years) in September/October 2014 (phase 1) and March/April 2016 (phase 2). Axial length (AL), corneal radii (CR) and spherical equivalent refraction (SER) were also recorded. Participants were defined as myopic if non-cycloplegic spherical equivalent refractive error ≤À0.50 DS at phase 1. Results: Nine participants were lost to follow up. Mel concentrations were measurable for all myopes (phase 1 n = 25, phase 2 n = 22) and non-myopes (phase 1 n = 29, phase 2 n = 23). SER did not change significantly between phases (p = 0.51). DA concentrations were measurable for fewer myopes (phase 1 n = 13, phase 2 n = 12) and non-myopes (phase 1 n = 23, phase 2 n = 16). Myopes exhibited significantly higher Mel concentrations than non-myopes at phase 1 (Median difference: 10 pg mL À1 , p < 0.001) and at phase 2 (Median difference: 7.3 pg mL À1 , p < 0.001) and lower DA concentrations at phase 2 (Median difference: 4.7 pg mL À1 , p = 0.006). Mel concentrations were positively associated with more negative SER (all r ≥ À0.53, all p < 0.001), longer AL (all r ≥ 0.37, all p ≤ 0.008) and higher AL/CR ratio (all r ≥ 0.51, all p < 0.001).Conclusion: This study reports for the first time in humans that myopes exhibit higher serum Mel concentrations than non-myopes. This may indicate a role for light exposure and circadian rhythm in the human myopic growth mechanism. Further research should focus on younger cohorts exhibiting more dynamic myopic progression and explore the profile of these neurochemicals alongside evaluation of sleep patterns in myopic and non-myopic groups.

PURPOSE To evaluate the agreement of cycloplegic refractive error measures between the Grand Seiko and Retinomax autorefractors in 4- and 5-year-old children. METHODS Cycloplegic refractive error of children was measured using the Grand Seiko and Retinomax during a comprehensive eye examination. Accommodative error was measured using the Grand Seiko. The differences in sphere, cylinder, spherical equivalent (SE) and intereye vector dioptric distance (VDD) between autorefractors were assessed using the Bland-Altman plot and 95% limits of agreement (95% LoA). RESULTS A total of 702 examinations were included. Compared to the Retinomax, the Grand Seiko provided statistically significantly larger values of sphere (mean difference, 0.34 D; 95% LoA, −0.46 to 1.14 D), SE (mean, 0.25 D; 95% LoA, −0.55 to 1.05 D), VDD (mean, 0.19 D; 95% LoA, −0.67 to 1.05 D), and more cylinder (mean, −0.18 D; 95% LoA, −0.91 to 0.55 D). The Grand Seiko measured ≥0.5 D than Retinomax in 43.1% of eyes for sphere and 29.8% of eyes for SE. In multivariate analysis, eyes with SE of >4 D (based on the average of two autorefractors) had larger differences in sphere (mean, 0.66 D vs 0.35 D; P < 0.0001) and SE (0.57 D vs 0.26 D; P < 0.0001) than eyes with SE of ≤4 D. CONCLUSIONS Under cycloplegia, the Grand Seiko provided higher measures of sphere, more cylinder, and higher SE than the Retinomax. Higher refractive error was associated with larger differences in sphere and SE between the Grand Seiko and Retinomax. (J AAPOS 2017;21: 219–223)

Purpose. To evaluate the agreement and repeatability between a new commercially available binocular open-field wavefront autorefractor, as part of the Eye Refract system, and a monocular closed-field wavefront autorefractor (VX110). Methods. A cross-sectional, randomized, and single-masked study was performed. Ninety-nine eyes of 99 healthy participants (37.22 ± 18.04 years, range 8 to 69 years) were randomly analyzed. Three measurements with the Eye Refract and the VX110 were taken on three different days, under noncycloplegic conditions. Mean spherical equivalent (MSE), cylindrical vectors (J0 and J45), and binocular corrected distance visual acuity (BCDVA) were compared between both autorefractors. An intersession repeatability analysis was done considering the values of repeatability (Sr) and its 95% limit (r). Results. The VX110 showed more negative values (P<0.001) in terms of MSE in comparison with the Eye Refract (0.20 D). Regarding cylindrical vectors, J45 showed statistically significant differences (P=0.001) between both wavefront autorefractors, but they were not clinically relevant (<0.05 D). In BCDVA, there were no statistically significant differences (P=0.667) between both wavefront autorefractors. Additionally, the Eye Refract was more repeatable than the VX110 in terms of both MSE (SrEYE REFRACT = 0.21 D, SrVX110 = 0.53 D) and J0 (SrEYE REFRACT = 0.12 D, SrVX110 = 0.35 D). Conclusions. The Eye Refract provided enough accuracy and reliability to estimate refractive errors in different age groups, achieving better results than the VX110. Therefore, the Eye Refract proved to be a useful autorefractor to be incorporated into clinical practice.
