Clinical features and management of autoimmune hepatitis

Abstract: Autoimmune hepatitis (AIH) is a chronic hepatitis of unknown etiology which can progress to cirrhosis. Its clinical manifestations are highly variable and sometimes follow a fluctuating course. Diagnosis is based on characteristic histologic, clinical, biochemical and serological findings. Anti-inflammatory/immunosuppressive treatment frequently induces remission but long-term maintenance therapy is often required. Liver transplantation is generally successful in patients with decompensated cirrhosis unrespons… Show more

“…Type 2 AIH is characterized by presence of anti liver kidney microsomal (LKM) and/or antiliver cytosol (LC1) antibodies. 8 Type 2 AIH is less common and tends to be more severe than AIH type 1. It shares most of the features with AIH-1, but occurs more commonly in children.…”

“…Seronegative AIH has typical appearance of AIH on histology, responds to immunosuppression, but lacks detectable autoantibodies . [18] This is a rare form of AIH in adults, but its prevalence and clinical characteristics remain to be defined in children.…”

“…It is possible that the herbal medicine may have caused bile duct toxicity, autoimmune hepatitis and resulted in transformation to malignant cells. However, it appears more plausible that the herbal remedy simply resulted in drug-induced hepatitis with histological findings that mimic autoimmune hepatitis [17] . The case also reinforces the suspicion that the association of autoimmune hepatitis with CCA may be more than mere coincidence.…”
