Objective This study intends to clarify the effect of regional disaster preparedness education on efforts to prevent the isolation of older people in the event of a disaster. Design and Sample This quasi‐experimental study involved participants aged 65 and above. The intervention group (n = 35) and the comparison group (n = 61) were compared in the first survey and another one month later. Measurements The survey items assessed aspects such as awareness of and actions related to isolation preparedness, and awareness of support needed by others in the event of a disaster. The results were compared between the two groups. Intervention The educational program was based on the transtheoretical model of health behavior change and focused on preventing isolation in the event of a disaster, by connecting vulnerable people with their neighbors. Puppet shows and group work were used as part of the program. Results A month after the program implementation, the intervention group had significantly higher awareness of and actions related to isolation preparedness than the comparison group. Conclusions Disaster preparedness education invokes a responsibility toward the prevention of isolation in the disaster and perceives it as a community issue and encourages individuals to support others.

The 2011 Great East Japan Earthquake (GEJE) has renewed the paradigm of disaster medicine. The Tohoku Journal of Experimental Medicine (TJEM) widened its scope to include the disaster science from the health perspectives. TJEM has been accumulating 76 articles related with "disaster" or "pandemic" out of which 69 were published after 2011. Tohoku University established the International Research Institute of Disaster Science (IRIDeS) that took initiative to impact the Sendai Framework for Disaster Risk Reduction 2015-2030 (Sendai Framework) to include health aspect. Sendai Framework provided the platform for collaboration of different sectors, including the terminology that defines the concept of disaster, risk, hazard & exposure, vulnerability and coping capacity. Disaster medicine covers the diverse causes and damages of disasters by various hazards in this globalized and rapidly changing world. TJEM articles range the physical and mental health damage after the GEJE and other disasters with approved ethical consideration of investigations from the view point of affected area, mechanisms of hazard to affect human health including the radiation, virus or hazardous materials, proposal of refinement of health system to cope with disasters such as mental health support, risk communication, disaster medical coordination and hospital business continuity plan and future perspectives with reconstruction including Tohoku Medical Megabank Project. TJEM scope on disaster medicine had been widened during the 10 years after GEJE and IRIDeS can be the bridging hub not only between the health sector and other sectors, but also between disaster medicine and other medical disciplines.

Purpose of Review Natural disasters occur frequently in Japan. A disaster medical system was rapidly developed in Japan following the Great Hanshin-Awaji Earthquake in 1995. Dentistry has become increasingly important in disaster medicine. This review summarizes the roles of dental professionals in disaster medicine, highlights relevant issues, and identifies new directions for research to improve disaster relief activities based on our previous experiences as dental professionals supporting the victims of major disasters. Recent Findings Many preventable deaths after a disaster are caused by aspiration pneumonia, which occurs against a background of factors that are compounded by a harsh living environment. An important aim of dental care in disaster medicine is to prevent these disaster-related deaths in vulnerable persons such as the elderly. This can be achieved through interventions to maintain oral hygiene, preserve and enhance oral function (i.e., chewing and swallowing), and improve the diet, since these interventions help to prevent the development of malnutrition and frailty in vulnerable people. Dental identification of disaster victims could be improved through the use of intraoral three-dimensional scanners and artificial intelligence to automate the acquisition of dental findings and through the construction of a national database of digitized dental records. Advances in personal identification methods will be needed given the prediction that a catastrophic earthquake will occur on the Nankai Trough during the next 30 years and claim more victims than the 2011 Great East Japan Earthquake. Summary Disaster-related deaths due to aspiration pneumonia can be prevented by providing appropriate dental care to those in need. The process of identifying victims could be made more efficient through the use of intraoral three-dimensional scanning, artificial intelligence, and a digital database of dental records. Establishing and strengthening relationships between professionals in different regions will help to optimize the multidisciplinary response to future large-scale disasters.
