Abstract: Vitiligo is the most common cause of depigmentation and its estimated worldwide prevalence ranges from 0.5% to 2%. The disease is characterized by the development of white macules resulting from a loss of epidermal melanocytes. The term vitiligo (nonsegmental) is now a consensus umbrella term for all forms of generalized vitiligo. ObjectivesTo detect ultrasonographic anatomical alterations in all the skin layers in patients with vitiligo.MethodsA prospective observational color Doppler ultrasound study was performed in nonsegmental face and/or neck vitiligo patients without a history of previous treatments. Two sites, a lesional area and a contralateral clinically healthy region, were ultrasonographically studied and compared in the same patient. All cases were studied in high‐frequency (24 MHz) and ultra‐high‐frequency (70 MHz) ultrasound devices with the highest axial spatial resolution available in the market. Demographic data of the sample, ultrasound grayscale, and color Doppler features were recorded and analyzed.ResultsTen patients met the study criteria (60% females; mean age 49 years). All cases presented ultrasonographic undulation of the epidermis in the affected zones vs 50% in the healthy control regions, being more prominent in the vitiligo areas. Eighty percent demonstrated intense hypoechoic thin plaques in the upper dermis (subepidermal).All vitiligo areas presented thickening and hypoechogenicity of the regional hair follicles and/or pilosebaceous units. Ninety percent showed prominent sebaceous glands, and 20% demonstrated a hypoechoic cap surrounding the sebaceous glands in the lesional areas.Dermal hypervascularity was detected in 100% of the affected regions and 40% of the clinically healthy areas.ConclusionUltrasound can identify subclinical inflammatory cutaneous patterns in the epidermis, dermis, hair follicles, pilosebaceous units, and sebaceous glands in vitiligo. This noninvasive information can support early detection, monitoring, and research, including the clinical trials of drugs used to manage this devastating disease.

BackgroundA 308 nm excimer light is an effective treatment for vitiligo, with fewer treatment times, rapid obvious effects, relatively complete pigment recovery, good patient compliance, and safe and effective treatment for children. The disadvantage of this treatment is that some patients have a plateau during which the skin lesion reduction rate slows down or stalls after multiple treatments.MethodsTreatment data were collected from all vitiligo patients in the Dermatology Department, distinguishing between those who underwent 308 nm excimer light treatment alone and those who received a combination of 308 nm excimer light and 2940 erbium laser treatment.ResultsOf the 104 patients, 60 were treated with 308 nm excimer light only, and 44 were treated with a combination of a 2940 erbium laser and 308 nm excimer light. In contrast to the treatment of 308 nm excimer light alone, the combined treatment of the 2940 erbium laser and 308 nm excimer light significantly increased the speed and degree of repigmentation in vitiligo.ConclusionOur study demonstrates that the combination of 308 nm excimer light and 2940 nm erbium laser treatment can improve the treatment response for vitiligo, decrease the occurrence of treatment plateaus associated with 308 nm excimer light, and improve efficacy.Trial RegistrationChiCTR2000036712 (24/08/2020)

Vitiligo is a chronic autoimmune skin disorder characterized by depigmentation due to melanocyte destruction, significantly impacting patients' quality of life. Emerging treatments, including Janus kinase inhibitors like tofacitinib, offer promising alternatives to conventional therapies such as corticosteroids. Objective: To compare the efficacy of tofacitinib with betamethasone pulse therapy in achieving re-pigmentation in vitiligo patients. Methods: This quasi experimental study was conducted on 42 patients of vitiligo of either gender with ages between 12 and 65 years and had a history of vitiligo for over one year with a body surface area affected by vitiligo exceeding 5%, and a vitiligo area scoring index score of more than 10 were included in the study. Patients were divided into 2 equal groups using alternate assignments. Group A were treated with betamethasone pulseerapy of 4mg twice a week. Group B were treated with tofacitinib at a dose of 5 mg twice a day. Results: Optimal recovery (vitiligo area scoring index decrease ≥20% from baseline) was observed in 14 (66.7%) of the tofacitinib group compared to 6 (28.6%) in the betamethasone group, highlighting tofacitinib’s superior efficacy in achieving significant vitiligo area scoring index reduction. Over three months, the BSA affected by vitiligo decreased in both groups, with a significantly greater reduction in Group B (tofacitinib) compared to Group A (betamethasone). Conclusions: It was concluded that tofacitinib may be more effective than betamethasone pulse therapy in reducing both the extent and severity of vitiligo.
