Multiple linear regression analysis is frequently used in studies investigating the degree of Functional Independence Measure (FIM) improvement in stroke patients. However, the coefficient of determination R 2 is about 0.46 to 0.73, meaning that the prediction accuracy is not necessarily high. In order to improve the prediction accuracy, the following methods are used; using appropriate explanatory variables, using FIM effectiveness which corrected the ceiling effect as the objective variable, creating multiple prediction formulas, converting numerical variable of explanatory variables into dummy variable, adding FIM improvement for one month to the explanatory variables. Even so, it is difficult to predict patients whose FIM gain is extremely large or small. It is desirable to combine these methods or develop new methods to achieve the accurate prediction.

Nutritional complications frequently occur among patients undergoing rehabilitation, and the importance of nutrition in these patients has been emphasized. However, there is not enough evidence available on rehabilitation nutrition. The Japan Rehabilitation Nutrition Database was set up to reflect the real-world clinical practice of rehabilitation nutrition. This paper describes the construction and quality evaluation of the registry database. We constructed a large-scale database that can be used for the clinical research of nutrition for rehabilitation. To verify the data, a simple comparison of the numbers of cases, data loss, data duplication, data type errors, out of range values, and input errors in the option columns was performed. From March 2016 to June 2017, 797 cases were registered in the rehabilitation database from 18 facilities. The variable entry error frequency ranges from 0% to 0.4% and the frequency of a main item missing from 0.4% to 10.9%. Energy intake on hospitalization was missing in 10.9% of cases, and Mini Nutritional Assessment Short Form and Food Intake LEVEL Scale on hospitalization was missing in 9.7% of cases. Stroke accounted for 45.7% of the diseases registered, pneumonia for 36.5%, and proximal femoral fracture for 17.8%. Through the use of this database, research on rehabilitation nutrition can be conducted, and there is a possibility that useful results for future clinical practice may be obtained. Verification of the secondary use of the database is becoming the basis for evidence-based nutritional rehabilitation.

Objective: To validate the International Classification of Functioning, Disability and Health (ICF) Core Set for stroke in the assessment of functional status and disability in Japanese stroke patients. Methods: The study included stroke patients admitted to the Kaifukuki (convalescent) rehabilitation wards. The comprehensive ICF Core Set for neurological conditions for postacute care and the ICF rehabilitation set were evaluated with qualifiers assessed by the physiatrists at admission. The "activity and participation" (d) component was divided to sub-components (cognition-related activity, motor-related activity and participation). The correlations between numbers of problem categories in the entire "d" component and these sub-components in each ICF Core Set and the Functional Independence Measure (FIM) score were assessed using Spearman's correlation coefficient. Results: A total of 117 post-stroke patients (mean age 70.1 ± 14.2 years, 53 women) were included. Correlation analysis identified significant and strong correlations between the values of the entire "d" component and sub-components (cognition-related activity and motor-related activity) of the 2 ICF Core Sets and FIM score. A significant, but weak, correlation between FIM and the participation subcomponent was identified. Conclusion: The "d" component of these 2 ICF Core Sets reflects functional status and disability and could be a valid measure in post-acute stroke patients in the rehabilitation setting.
