Background: Traditional rubber band ligation can improve the symptoms of hemorrhoids, the techniques used vary among centers and the degree of hemorrhoids may also affect the therapeutic efficacy and postoperative outcome, especially for patients with grade III hemorrhoids (hemorrhoid prolapses). This study aimed to investigate the clinical efficacy of modified rubber band ligation (MRBL) in the treatment of grade III internal hemorrhoids.Methods: A total of 120 patients with grade III internal hemorrhoids were randomly assigned to receive MRBL or Milligan-Morgan haemorrhoidectomy (MMH) (n=60 per group). The post-operative pain, bleeding, urine retention and feeling of anal distension were recorded, and the resting anal pressure (RAP) and post-operative recurrence rate were compared between two groups.Results: The post-operative pain, bleeding and urine retention in the MRBL group were improved significantly as compared with the MMH group (P<0.05), but the feeling of anal distension was similar between them (P>0.05). The RAP remained unchanged after MRBL, but the RAP at 1 month after surgery in the MMH group increased markedly (P<0.01) as compared with that before surgery and was significantly higher than that in the MRBL group (P<0.01). The post-operative recurrence rate was comparable between two groups (P>0.05).Conclusions: As compared with traditional MMH, MRBL is effective to attenuate the post-operative pain and other discomforts and stabilize the RA. Thus, MRBL is an ideal choice for the treatment of grade III internal hemorrhoids.

Grade II-III hemoroidal hastalığı olan toplam 146 hasta retrospektif çalışmamıza dahil edildi. Operasyon süresi, uygulanan ligasyon sayısı, postoperatif 7. gün vizüel analog skala değerleri, komplikasyonlar ve 6. ay sonunda semptomsuz hasta sayıları her iki işlem için istatistiksel olarak karşılaştırıldı. Bulgular: HAL yöntemi uygulanan hastaların LBL uygulanan hastalara göre daha erken işe döndüğü ve postoperatif 7. gün daha az ağrılarının olduğu ve LBL yönteminin ise daha düşük maliyete ve kısa operasyon süresine sahip olduğu istatistiksel olarak saptandı. Semptomsuz hasta oranları 6. ay sonunda her iki yöntem için de benzer olarak (%94 ve %91,66) saptanmış olup istatistiksel olarak fark yoktu. A total of 146 patients with grade II-III hemorrhoidal disease were included in this retrospective study. Surgery duration, performed ligations, pain on postoperative day 7 visual analog scale, complications, and ratio of symptom-free patients at 6 months were statistically analyzed for both procedures. Results: Patients who underwent HAL returned to work earlier and had less pain on postoperative day 7 than those who underwent RBL; in addition, the RBL procedure was statistically more cost-effective and had shorter surgery times. There was no statistical difference between the HAL and RBL groups in proportion of patients who were symptom-free at postoperative 6 months (94% and 96.6%, respectively). Conclusion: The RBL and HAL procedures are both effective and have low complication, and can be used safely together or separately in the treatment of grade II-III hemorrhoidal disease.

tarihleri arasında kliniğimizde hemoroid şikayeti nedeniyle LBL uygulanan hastaların kayıtları incelendi. Verileri eksik olan veya tam olarak kayıtlarına ulaşılamayan hastalar çalışmaya alınmadı. Bulgular: Polikliniğimizde 25 hastaya lastik band ligasyonu uygulandı. İki hastanın verilerine tam ulaşılamadığı için çalışma 23 hasta üzerinden yapıldı. Hastalardan 10' u kadın, 13' ü erkekti. Yaş ortalaması 33,4 (18-67) idi. LBL semptomatik 2. ve 3. derece hemoroidal hastalığı olan ve cerrahi dışı yöntemlerle tedavi olmak isteyen hastalara uygulandı. 15 hastada grade 2, 8 hastamızda grade 3 hemoroit pakesine işlem yapıldı. 16 hastada (% 69) kanama, 6 hastada (% 26) prolapsus, 8 hastada (% 35) dışkılama zorluğu, 14 hastada (% 61) ağrı ve 11 hastada da (% 48) kaşıntı şikayetleri vardı. LBL uyguladığımız hastalarımızdan 1 olguda ağrı ve 1 olguda da minimal kanama gözlendi. Sonuç: Sonuç olarak LBL yönteminin güvenli, etkili, kolay uygulanabilen bir yöntem olması ve ayaktan tedavi protokolü olduğu için hemoroidin cerrahi dışı tedavilerinden en sık kullanılan tedavi yöntemidir. Fakat bu yöntemin uygulanma sonrasında düşen hemoroid pakesinin mikroskopik tetkikinin yapılamaması bir dezavantaj olarak görülebilir.
