Clinical Outcome of IBD-Associated versus Sporadic Colorectal Cancer: A Matched-Pair Analysis

Abstract: We demonstrate for the first time survival differences between IBD-CRC and SCRC after elimination of five known confounders. This might be caused by a difference in tumor biology resulting in a higher local recurrence rate in IBD-CRC.

“…The negative effect of UC on the prognosis of CC was more pronounced in the first year after cancer diagnosis, in patients diagnosed with CC before the age of 70 years, in patients with UC duration of eight years or more, and in cancers with advanced or unknown stage [18]. In a recent matched pair analysis, 33 patients with inflammatory bowel disease-associated CC had a significantly reduced and markedly lower OS rate, as well as a notably increased recurrence rate, compared to sporadic CRC patients (48.6 vs. 67.1%, respectively) [22]. A single-centre retrospective study performed in 2016 compared OS and surgical technique in 27 patients with UC and rectal cancer and 54 patents with sporadic rectal cancer [14].…”

“…Similar to our methodology, patients were matched on age, gender, tumor stage, tumor site, and date of surgery. These investigators found that IBD was independently associated with a significantly higher local tumor recurrence (30.3% vs. 10.8%, P ¼ 0.004) and significantly lower 5-year overall survival (49% vs. 67%, P ¼ 0.03) [21]. They also observed that patients with IBD had a significantly higher proportion of high-grade tumors compared to those with sporadic disease, though on further analysis this did not seem to account for the difference in local recurrence rates.…”

“…The prognosis of CRC has been described to be poorer in patients with IBD 10 31 . Beside a higher complication rate, significantly higher local recurrence and lower 5-year-survival rates were observed for patients with IBD-related cancer.…”

“…Recently, the groups of Renz and Scaringi could show lower 5-year survival and higher local recurrence rates in inflammatory bowel disease (IBD)-related compared to sporadic CRC and suggest a link to the differences in tumour biology 10 11 .…”
