Clinical Outcome of Surgical Hip Dislocation Combined with Impacting Bone Grafts and Implanting Iliac Bone Flaps in the Treatment of Osteonecrosis of the Femoral Head: A Mid‐term Retrospective Study

Abstract: Objective To report the medium‐term outcomes of surgical hip dislocation (SHD) combined with impacting bone grafts and implanting iliac bone flaps in the treatment of osteonecrosis of the femoral head (ONFH) and to define the indications for this treatment. Methods This was a single‐center retrospective study. In total, 64 patients (70 hips) with ONFH treated from January 2014 to December 2017 were included in this study. There were 51 males and 13 females aged 18–55 years with an average age of 32 years. All … Show more

“…The main reason was ARCO system may focus more on the extent of the collapse rather than the regional distribution. As reported by our previous studies [ 15 , 22 ], in patients with Japanese Osteonecrosis Investigation Committee (JIC) classification type C2, the absence of the lateral column of the femoral head as the main weight-bearing site makes ONFH more likely to progress. However, the information could not be meta-analyzed due to lack of available data from the included studies.…”

“…Then, the necrotic bone tissue was completely removed, and the void can be packed by a cortico-cancellous graft or augmented as needed. Our recent study [ 15 ] reviewed 64 patients who underwent surgical hip dislocation combined with impacting bone grafts. Patients had between ARCO stage II and III ONFH.…”

“…Secondly, both procedures require attention to the issues of intraoperative dead bone clean-up, over-implantation of compression and postoperative length and extent of cartilage and implanted bone. As the maximum equivalent force in the mechanical analysis of both patients A and B appeared in the cortical bone at the periphery of the femoral stem, the conduction of internal femoral head stresses was visible in the section, but a continuous conduction structure was not formed 9 .…”
