Clinical outcomes after salvage radiotherapy without androgen deprivation therapy in patients with persistently detectable PSA after radical prostatectomy: results from a national multicentre study

Abstract: In patients with persistently detectable PSA after RP and selected for local salvage treatment, SRT offers good oncologic clinical outcomes. The most powerful pathologic predictive factors of SRT failure include a pT3b stage, a Gleason score 8 or more cancer and high PSAV and pre-SRT PSA levels. Patients having a high PSAV >0.04 ng/mL/mo would be potentially better candidates for a systemic therapy due to a high SRT failure rate.

“…Prostate serum antigen (PSA) level measurement is commonly used to monitor these patients. After treatment, PSA concentration falls very slowly with the nadir, reached after 2–4 years . A rise of PSA serum level of 2 ng/ml above the nadir defines biochemical recurrence (Phoenix criteria) .…”

“…Помимо вышеописанных исследований, которые доказывают важное прогностическое значение величины ПСА на момент начала СЛТ, представлено достаточное количество других научных работ в мировой литературе по изучению этого вопроса [8][9][10][11][12]. Главный посыл данных публикаций заключается в том, что СЛТ следует начинать при наиболее низких значениях уровня ПСА (менее 0,5-1,0 нг / мл).…”

“… Definition: Recurrence post-RP is defined by PSA level >0.2 ng/mL in two consecutive readings[ 61 62 63 64 65 ] Factors helping to differentiate local relapse or distant metastasis are: The timing of PSA recurrence, PSA doubling time (PSADT), pathological stage, and final Gleason score[ 66 67 ] Treatment of local recurrence is early salvage EBRT preferably with ADT, which results are improved if given with lower PSA value (<0.5)[ 68 69 70 71 72 73 74 75 76 77 78 ] In biochemical recurrence, bone scan and CT are of no diagnostic value unless PSA value is higher than 20 ng/dL,[ 79 80 81 ] Gleason >7, or clinically indicated (EL-2). …”
