Purpose: To evaluate the clinical, radiographic, and prosthetic outcomes of Poly Ether Ether Ketone (PEEK) versus Poly Ether Ketone Ketone (PEKK) frameworks in mandibular implantsupported fixed detachable prosthesis. Materials and method:Fourteen completely edentulous patients received complete dentures for three months. Four implants were inserted in the mandible at the canine and molar-premolar areas bilaterally following two-stage surgical technique. All patients were planned to receive mandibular fixed-detachable prostheses with individual zirconia crowns. The patients were randomly allocated to two groups according to the prosthesis framework material . Group A received a prothesis with PEEK framework while group B received a prothesis with PEKK framework. The implant success rate, modified Plaque Index (mPI), simplified Gingival Index (sGI), modified Sulcus Bleeding Index (mSBI) and Peri-implant bone loss (PIBL) were evaluated at loading, 6-and 12-months follow-up visits. Additionally, the total prosthetic complications were measured after a one-year follow-up period.Results: All implants had a 100% success rate. The PEEK group showed statistically significant higher mPI, sGI and mSBI compared to the PEKK group at the 6 and 12 follow-up visits. Regarding PIBL results, no statistically significant difference was found between the two groups. The prosthetic complications in the PEEK and PEKK groups were four and eight events respectively. The most frequent complication was abutment screw loosening. ConclusionWithin the limitations of this study, the fixed-detachable implant-supported prosthesis with PEEK or PEKK framework can be a clinically successful treatment option for the rehabilitation of edentulous mandible. However, the fixed-detachable prosthesis with the PEKK framework may provide superior soft tissue health outcomes and fewer prosthetic complications compared to those with the PEEK framework.

Purpose:The aim of this study was the evaluation of using pre-adapted titanium mesh on 3D-model with bovine bone graft and platelet rich fibrin for augmentation of posterior atrophic mandibular ridge.Patients and methods: Prospective study was conducted on eight (8) patients with posterior atrophic ridge using the Pre-adapted Titanium mesh with bovine bone graft and platelet rich fibrin, then implant installation simultaneously after removing Ti-mesh followed by the insertion of Final restoration. Then, by CBCT evaluated the volume of bone gain in height and width also, Implants were evaluated clinically and radiographically.Results: Six months post-augmentation, CBCT evaluation revealed a mean horizontal bone gain of 4.828 mm (± 0.378), and the mean vertical bone gain of 3.822 mm (± 0.828). The most frequent complications were mild postoperative edema and discomfort after surgery; these complications were resolved within one week. Titanium mesh exposure occurred in one (1) patient (1/8: 12.5%): this suffered partial loss of the graft. An implant survival rate of 100 % (implant-based) and a periimplant marginal bone loss of 0.521 mm (±0.15) were recorded after 1 year. Conclusion:This study suggested that using pre-adapted Ti-mesh and bovine bone graft was considered as optimal treatment modality for augmentation atrophic mandibular ridge.

To compare the clinical, radiographic, and prosthetic outcomes of zirconia-polyether ether ketone (PEEK) versus acrylic resin-metal mandibular full-arch fixed detachable implant prostheses (FDIP) during a two-year follow-up. Materials and methods: Twelve completely edentulous participants were selected. For each patient, six implants were installed in the mandible using a flapless surgical technique. Participants were randomly assigned to one of two groups: zirconia-PEEK FDIP or acrylic-metal FDIP. The clinical and radiographic parameters were assessed at prosthesis insertion, then after one and two years. The prosthetic complications were recorded from the time of prosthesis insertion for two years. Results: There were statistically significant differences in plaque and gingival indices as well as probing depth between the two groups, where the acrylic-metal group showed higher clinical outcome values than the zirconia-PEEK group. The zirconia-PEEK group showed significantly higher crestal bone loss than the acrylic-metal group. During the whole followup period, there were (n = 36, 62% of the total complications) prosthetic complications in the acrylic-metal group and (n = 22, 38 % of the total complications) in the zirconia-PEEK group. The total number of prosthetic complications was not significantly greater in the acrylic-metal group than in the zirconia-PEEK groups. There were no fractures of the prosthetic framework or abutment in either group. Conclusion:Within the limits of this study, zirconia-PEEK full-arch FDIP produces better clinical and prosthetic outcomes than acrylic-metal FDIP. However, zirconia-PEEK FDIP may lead to higher crestal bone loss than acrylic-metal FDIP.
