Clinical Pharmacists in Chronic care [Part 1]

Abstract: Figure 1: Clinical pharmacy offers chronic care services like asthma, diabetes, nutritional supplement counseling, smoking cessation, weight reduction, geriatric care, hyperlipidemia, hypertension, naturopathy and wound care. The integration of clinical pharmacists into primary care clinics could have positive effects on the clinical outcomes of patients in glycemic control, blood pressure, lipid profile, in accordance with current guidelines.
