Clinical practice guideline for cognitive impairment of cerebral small vessel disease

Abstract: Cognitive impairment of cerebral small vessel disease (CSVD) is one of the most common cognitive disorders. It has a high incidence and results in heavy social burden; thus, it is essential to provide reasonable diagnosis and treatment in clinical practice. Based on the results of clinical research and related reports, combined with the actual situation in China, we propose a diagnosis and treatment guideline for cognitive impairment of CSVD.

“…CSVD is common in the population, especially in the elderly, and is implicated in up to a quarter of all strokes (Suzuyama and Yakushiji, 2020). Additionally, CSVD is widely regarded as a common cause of cognitive decline and dementia (Peng, 2019).…”

“…At present, according to the guidelines related to VCI, CSVD is diagnosed by aetiology or classification. 32 Therefore, according to current clinical practice guidelines for cognitive impairment of CSVD in China (2019), 2 the diagnosis of cognitive impairment of CSVD must be met: (i) the establishment of the presence of cognitive impairment and (ii) the determination that CSVD is the dominant pathology that accounts for the cognitive deficits. The specific diagnostic criteria for cognitive impairment due to CSVD are listed in table 1 .…”

“… 1 More importantly, with the accelerated ageing of the population and increased incidence of CSVD, cognitive impairment of CSVD is one of the most common cognitive disorders. 2 Approximately 30%–64% of patients with CSVD have different degrees of cognitive impairment. 3 CSVD-induced vascular cognitive impairment (VCI) is the second leading cause of dementia after Alzheimer’s disease.…”

“…Up till now, there is still no globally recognized diagnostic criteria for CSVD-VCI, and therefore, our review will include patients diagnosed with CSVD-VCI according to diagnostic criteria in the international randomized controlled trials[ 59 60 61 62 ] or Clinical Practice Guideline for Cognitive Impairment of Cerebral Small Vessel Disease proclaimed by Geriatric Neurology Group, Chinese Society of Geriatrics in 2019. [63] Briefly speaking, these diagnostic criteria contain 3 key points: subjective cognitive decline and objective evidence of cognitive impairment, neuroimaging evidence of CSVD, and identification of CSVD as evidence of cognitive impairment (namely, exclusion of cognitive impairment caused by other diseases). In particular, included studies should pay attention to the cognitive change of patients through detecting their cognitive function before and after treatments, and use at least 1 globally standardized and validated clinical scale to quantify this change, as one of the outcomes.…”
