We reviewed the cases of 100 patients (mean age, 73 ± 10 yr; 64 men) who had mitral valve replacement with a Medtronic Mosaic porcine bioprosthesis from 1995 through 2011. The mean New York Heart Association (NYHA) class was 3 ± 0.7, and 52 patients were in atrial fibrillation. Prosthetic sizes were chiefly 27 mm (50 patients) and 29 mm (40 patients). Follow-up ended in December 2012 and is 97% complete, with a cumulative duration of 611 patient-years (mean duration, 6 ± 4.6 yr; maximum, 17.7 yr). The early mortality rate was 10% (6% in elective patients); late deaths occurred in 31 patients (5 valve-related). Actuarial survival rates at 5, 10, and 15 years were 74% ± 5%, 50% ± 6%, and 37% ± 8%. The mean NYHA class in survivors was 1.4 ± 0.6 (P <0.0001). Thromboembolic episodes occurred in 4 patients, with an actuarial freedom at 15 years of 91% ± 5%. No cases of endocarditis were observed. Four patients needed reoperation, 2 for structural failure, and 1 each for perivalvular leakage and valve thrombosis. Actuarial freedom from structural failure and from reoperation, respectively, was 93% ± 5% and 91% ± 5% at 15 years. Echocardiographic follow-up in 24 patients with 27-mm prostheses showed a mean gradient of 5 ± 1.7 mmHg and an effective orifice area of 1.57 ± 0.3 cm2; in 16 patients with 29-mm prostheses, the mean gradient was 4.5 ± 1.9 mmHg, and the effective orifice area, 1.63 ± 0.4 cm2. During nearly 17 years of follow-up, the Mosaic bioprosthesis has shown good overall clinical and hemodynamic performance after mitral valve replacement.

ПРИОБРЕТЕННЫЕ ПОРОКИ СЕРДЦАСовременные общемировые тенденции в области хи-рургического лечения приобретенных пороков сердца де-монстрируют значительное увеличение количества им-плантаций биологических клапанов по отношению к ис-пользованию механических протезов [1,12]. Данное об-стоятельство связано с изменениями в структуре пороков и соответственно увеличением среднего возраста опери-руемых пациентов, а также возможностью обеспечения лучшего качества их жизни за счет отказа от перманентно-го использования антикоагулянтной терапии при условии Объем наблюдения -213,9 пациенто-лет. Почти в 6% случаев оперативные вмешательства носили повторный характер, в 80% -были сочетанными. Результаты. Линеаризованный показатель отдаленной выживаемости составил 3% на пациента в год. Актуарные показатели общей и кардиальной выживаемости к 5-му году наблюдения составили 84,7 и 97,8% соответственно. Актуарный показатель отсутствия реопераций к 5-му году был равен 98,5%, отсутствие протезного эндокардита наблюдалось в 98,5% случаев. Госпитальная летальность при реоперациях отсутствовала. Случаев первичной тканевой несостоятельности, тромбоэмболий и кровотечений в течение 5 лет наблюдения выявлено не было. Выводы. Применение биологического протеза ЮниЛайн у пациентов 65 лет и старше обеспечивает высокую выживаемость к 5-му году наблюдения, сопоставимую с результатами применения современных зарубежных моделей биологических протезов клапанов сердца. Биологический протез ЮниЛайн имеет высокую толерантность к инфекциям. Имплантация биопротеза ЮниЛайн приводит к полноценной коррекции внутрисердечной гемодинамики: уменьшению размера левого предсердия и снижению систолического давления в легочной артерии при наличии синусового ритма. Даже частичное сохранение подклапанных структур достоверно улучшает течение процесса ремоделирования миокарда левого желудочка при коррекции митрального порока. Ключевые слова: биологический протез клапана сердца, митральный порок.Aim. To estimate immediate results of mitral valve replacement for acquired valve disease using Uniline bioprosthesis. Material and methods. Since January 2009 to September 2014 in the Research Institute for Complex Issues of Cardiovascular Diseases (Kemerovo) Uniline bioprosthesis was implanted into mitral position in 85 patients (mean age 64.8±8.5 years). Average follow-up was 2.6±1.8 years (range 0.2-5.4 years). Observation's volume is 213.9 patient-years. Repeated and combined interventions were performed in almost 6 and 80% of cases respectively. Results. Linearized long-term survival was 3% patient-years. Actual overall and cardiac 5-year survival were 84.7 and 97.8% respectively. Actual 5-year reoperation-free index is 98.5%, 5-year absence of prosthetic endocarditis was observed in 98.5% of cases. There was no in-hospital mortality in case of reoperations. Primary tissue failure, thromboembolism and bleedings have not been observed for 5-year follow-up. Conclusions. 1. The discussion begins with a thorough review of computational modeling and the governing equations of fluid and structural interaction. We then move onto multiscale and disease specific modeling. Finally, advanced methods related to in vitro testing of the heart valves are reviewed. This section of the review series is intended to illustrate application of computational methods and experimental studies and their interrelation for studying heart valves.
