Clinical Study of Chronic Pancreatitis with Focal Irregular Narrowing of the Main Pancreatic Duct and Mass Formation: Comparison with Chronic Pancreatitis Showing Diffuse Irregular Narrowing of the Main Pancreatic Duct

Abstract: These results indicate that CP with focal narrowing of the MPD is part of the same clinical spectrum as CP with diffuse narrowing of the MPD, and whether the distribution is diffuse or focal seems to be related to the stage or the extent of the disease. It is therefore important to recognize the possible existence of this focal variant to avoid unnecessary surgery.

“…However, clinical and instrumental improvement was also observed in patients who underwent derivative surgery (choledochoduodenostomy) and in non-treated patients (no jaundice at onset of the disease). These data are clearly affected by a selection bias, but they demonstrated that AIP may recover spontaneously in some patients, at least clinically, as previously reported [17,21].…”

“…However, we do not know the long-term effects of surgery on exocrine and endocrine pancreatic function and we cannot exclude the possibility of involvement of the residual pancreas after surgery. Some patients suffering from AIP have recurrences of the disease after resection [14][15][16][17].…”

“…In addition, since the first report by Sarles et al [ 41 ], no pseudocyst or calcification has been demonstrated in AIP, a characteristic being also one of the diagnostic criteria for IBD-related pancreatitis [37,41] . Histological changes demonstrated in patients with AIP-or IBD-related patients who underwent surgical resection or biopsy because of the pseudotumorous presentation were similar [2,4,5,42] . The pathogenesis of IBD-related pancreatitis remains unknown.…”

“…12,13 On pancreatography, a diffusely irregular, narrow pancreatic duct is highly suggestive of AIP. 13,14 However, some patients may have focal masses or strictures, [15][16][17] or show evidence of calcifi cation 18 or acute pancreatitis. 19,20 Serology Serological abnormalities in AIP include elevated titers of gammaglobulins, IgG, and a variety of antibodies including antinuclear antibody, rheumatoid factor, carbonic anhydrase, and lactoferrin.…”
