Objective: To compare the outcomes of frontalis sling with tarsal fixation or without tarsal fixation suture in patients of severe ptosis with poor levator function. Methods: This QUASI-experimental study was conducted at the College of Ophthalmology & Allied Vision Sciences/King Edward Medical Study after obtaining ethical approval vide no 2162/22. Ten Patients above 10 years of age and having levator muscle function of less than 4mm were selected by probability consecutive sampling. Those patients who were diagnosed with Blepharophimosis, Marcus Gun phenomenon, CN III palsy and other lid or facial deformities were excluded. Patients were equally divided into two groups A and B. Group A patients underwent fascia lata sling with tarsal fixation while Group B patients underwent simple fascia lata sling without tarsal fixation. Postoperatively (1 week) surgical success including marginal reflex distance (MRD) and complications including slippage of the sling, exposure keratopathy, recurrence and lid margin deformity were evaluated. Paired t-test and Independent sample t-test were applied to check for statistical significance. Results: Eight out of ten patients had bilateral ptosis with poor levator muscle function while the other two patients had unilateral ptosis. Changes in the marginal reflex distance and palpebral fissure height after surgery were statistically significant. Both marginal reflex distance and palpebral fissure height were statistically better in group A as compared to group B (0.001 & 0.003) respectively. Conclusion: Cosmetic results are better when a fascia lata sling is done with tarsal fixation sutures. Keywords: Ptosis, Fascia lata, Patients

Summary Acquired eyelid ptosis in adults, with complete loss of levator palpebrae superiosis function, can be a challenging problem to diagnose and treat. A 48-year-old woman with chronic bilateral severe blepharoptosis of 10 years duration is presented, whose neurological investigations excluded myasthenia gravis. The patient was preliminarily diagnosed with chronic progressive external ophthalmoplegia. The levator excursion was negligible, and a frontalis suspension procedure was considered using a conventional autogenous fascia lata graft. An optimal outcome was achieved with over 16-years follow-up. Although the patient was healthy otherwise upon first presentation, 10 years later, she developed other neurologic manifestations, including dysphagia and oral dryness. The fact that blepharoptosis did not recur over the years in this case differentiates an oculo-pharyngeal type of muscular dystrophy in this patient from other types and from the more frequent condition of chronic progressive external ophthalmoplegia.

Background Congenital ptosis is a drooping of one or both eyelids at birth, o en due to poor development of the levator palpebrae superioris muscle. This can result in amblyopia, astigmatism, and ocular torticollis and therefore may necessitate surgical intervention in early childhood if visual development is compromised. Patients may have varied levels of levator function. Those with moderate to good function may elect to first attempt ptosis repair with external levator advancement or mullerectomy/Fasanella-Servat procedures. For those with poor function, those procedures are less likely to be e ective, so they may undergo frontalis sling surgery, in which the tarsal plate is coupled to the frontalis muscle, so that movement of the brow and forehead result in movement of the eyelid. The optimal material to use in this surgery is unknown. Objectives To evaluate the comparative e ectiveness and safety of various materials used in frontalis sling surgery for congenital ptosis.
