Clinical trials in low and middle-income countries — Successes and challenges

Abstract: Gynecologic malignancies affect women in low and middle-income countries (LMICs) at equal or higher rates compared to high income countries (HICs), yet practice guidelines based on clinical trials performed in HICs do not routinely account for resource disparities between these regions. There is a need and growing interest for executing clinical trials in LMICs. This has led to the creation of multinational cooperative groups and the initiation of several ongoing clinical trials in Mexico, China, and Korea. In… Show more

“…Although much of the international funding is directed towards human papilloma vaccination and screening research, dedicated funding and new trials will be needed to improve therapeutics in women with cervical cancer. There are several challenges in developing research protocols in LMICs [81], however; the development of international collaborative groups can help to overcome some of these challenges. One such example is the Cervical Cancer Research Network (CCRN), a global networking effort by the Gynaecologic Cancer Intergroup.…”

“…One reason for this lack of research is the persistence of many barriers to clinical trial design and execution in LMICs 7. Research studies designed for and carried out in the developed world may not successfully apply in other populations or settings.…”

“…Even though RCTs are considered to be the gold standard for effectiveness studies [ 32 ], the ability to conduct such trials in LMICs are severely hampered by several logistical challenges [ 32 , 33 ]. For example, in resource-constrained settings, there is a paucity of facilities, trained human resources, and expertise.…”

“…For example, in resource-constrained settings, there is a paucity of facilities, trained human resources, and expertise. Thus, the execution of randomized trials is hindered and limited by both high costs and time requirements [ 32 , 33 ]. Furthermore, employing RCTs in complex public health interventions may be inappropriate, misleading, or unnecessarily expensive [ 33 ].…”
